I gave Jeremy Caldwell of PAC Development, the facility operator for PAC, and the Smith Transport brine truck driver a verbal warning that the UIC number must be displayed at all times. I informed Mr. Caldwell not to accept brine truck without UIC numbers.

0388693362 2018-02-02 Salt Water Injection Well No Violations I was on location in reference to the report of a strong odor coming from the UIC facility. The odor from the facility was minimal and only noticeable in close proximity of the Off-loading storage pit on the day of my inspection. I observed no spills or leakage from the diked facility or Off-loading storage pit.

I spoke with Steve Yendrich, PAC Development facility operator. Mr. Yendrich stated: -562) delivered a load of brine which contained approximately five (5) bbl. of condensate. -loading storage pit, odor from the condensate was detected by the general public on SR 422.

I obtained a copy of the manifest from the brine delivery from Spirit Services. Also, PAC Development is in the process of cleaning the Off-loading storage pit, the 13 septic tanks, and the storage tanks. There are approximately 34 roll-off containers with covered tarps on location. The clean-up operations are being performed by Odyssey.

I spoke with the facility operator. The facility operator stated they have been receiving light water. I instructed the facility operator to keep the injection well shut down until the pressures have dropped below the maximum allowable pressure. A follow-up inspection will be conducted on Monday, January 28, 2019.

1150906822 2018-08-07 Salt Water Injection Well No Violations I was on location to investigate PAC having numerous roll-offs at the UIC facility. I found nine (9) vac boxes and a roll-off with a tarp on the two (2) new concrete pads. Also, there are four (4) vac boxes next to the off-loading pad. I spoke with Steve Yendrich, facility operator on location. Mr. Yendrich stated Austin Masters shut down operations last Wednesday, 8/1/2018, and did not re-open until yesterday, 8/6/2018. During this time period, PAC was in the process of cleaning the sludge from inside the off-loading pit and a 400 bbl. storage tank. Due to the shut-down of Austin Masters, PAC requested a few extra vac boxes while clean-up operations were on going. Mr. Yendrich stated there are three (3) more 400 bbl. storage tank left to clean out prior to winter. Mr. Yendrich said Austin Masters has resumed removing the vac boxes. According to Mr. Yendrich, Austin Masters is only using one (1) truck to move the vac boxes making it a slow process. Also, I spoke with Justin Caldwell from PAC. Mr. Caldwell stated PAC normal has six (6) to eight (8) vac boxes on location. Follow-up inspections will be conducted.

1185048382 2018-08-09 Salt Water Injection Well No Violations I was on location for a follow-up inspection. I found PAC cleaning one of the 400 bbl. storage tanks. There are still 14 vac tanks still on location. I spoke with Jeremy Caldwell from PAC on location. Mr. Caldwell stated he is working at reducing the number of vac tanks on location to no more than eight (8) by the middle of next weeks (August 15, 2018). Mr. Caldwell stated there will be two (2) more storage tank left to clean prior to winter. I observed no spills or leaks. While on location, a Eureka Resources brine truck and three (3) Contractor Transport brine trucks arrived on location with the proper UIC numbers displayed (25-641 and 25-670).
1201515533 2015-12-03 Salt Water Injection Well No Violations I was on location in reference to complaint ID 5197. During my inspection, I found an identification sign posted above a window on the office building next to the tank battery. The emergency contact and number were incorrect. There is an identification sign posted at the well with the name and permit number. A locked, wooden fence surrounds the well. I instructed KDA to correct the emergency contact and number immediately. Next, I called and left a message with Ms. Beiersdorfer for a return call. A follow-up inspection will be conducted by 12/10/2015.

1428508885 2015-04-08 Salt Water Injection Well No Violations I inspected this salt water injection well as a general, routine status check. The facility is comprised of an aboveground storage tank (AST) battery with 9 210-barrel steel tanks in a common containment pad. Adjacent to the tank impoundment, is a combined maintenance/office/control room. The wellhead is located 320 feet behind (southwest) of the control building. Unloading is executed by attaching 4-inch flex-hose to cam-lock fittings at the pit area and fluids are drained into this open-topped, cement-sided impoundment structure which is underlain by a series of pre-cast cement tank vaults. Fluids are moved from the pit into the battery tanks via fluid pumps with flexible hoses. Secondary containment at the battery is comprised of both cement walls and berms. The berms consist of gravel-ballasted flexible liner over coarse-granular or gravel dike. Areas surrounding the AST impoundment and control building consist of parking and drive areas which are gravel and/or cinder-paved. The areas beyond the footprint of the facility are comprised of woods and low-lying marshy and bog-type terrain. I inspected the low-lying marshy and bog areas surrounding the facility and did not observe any sheens, free-oil, stressed vegetation, or other signs of leakage or spills. I observed the drainage culvert along the roadway at the entrance to be similarly free from staining, sheens, stressed vegetation, or other signs of impact. The well is located in a locked, board-on-board enclosure in a small clearing behind the facility. I observed connections and fittings at the wellhead to be secure and free from signs of escaping gases, fluids, or signs of leakage. At the time of my inspection, fluid was being off-loaded from a tank truck into the pit. At the time of my inspection, the interior of the impoundment was dry. I observed the horizontal and vertical surface areas of the dike structures to be free from staining. Exterior surface areas beyond the containment walls and berms was fre

A pressure of 1140 pounds per square inch (psi.) was placed on the tubing/casing annulus. There was a pressure loss of 10 psi. observed during the fifteen minute test period. The well passed the mechanical integrity test.
1502281984 2014-07-25 Salt Water Injection Well Violations Noted An inspection found the well exceeding the maximum allowable injection pressure. The well tender was instructed to slow the pump rate, lowering the pressure by more than 20 psi. on the tubing. The well owner was instructed to put a minimum pressure of 200 psi. on the tubing/casing annulus.
1515788291 2018-01-12 Salt Water Injection Well No Violations I was on location to inspect the UIC facility. I found the injection well pressure over the maximum allowable pressure by 30 psi. The soil and gravel around the injection well had oil residue. An identification sign is posted at the well and facility. I told the facility operator to immediately reduce the pressure. The facility operator stated the injection has been shut down due to the pressure and the area around the well impacted by the oil residue will be cleaned up. A follow-up inspection will be conducted by 1/18/2018.
1518611511 2018-02-12 Salt Water Injection Well No Violations I was on location to assess the progress of PAC Development, LLC with the removal of the roll-off containers and proper disposal of the waste material. I met with Justin, Jason, and Jeremy Caldwell of PAC Development officials and Jack Bement, CEO of Austin Master. Odyssey was on location cleaning the facility. The PAC officials and Mr. Bement inspected all 34 roll-off containers in preparation for proper removal. PAC officials estimated 2/14/2018 as the start date for the removal the roll-off contains. Once the roll-off contains are removed, PAC officials will clean-up and properly dispose of all waste material impacting the ground surface. Continue inspection will be conducted.
1518619311 2018-02-14 Salt Water Injection Well No Violations I was on location to assess the progress of PAC Development, LLC with the removal of the roll-off containers and proper disposal of the waste material. I found one (1) vac box has been removed by Ground Tech and one (1) roll-off container has been removed by Wolford. Both containers were taken to Austin Masters. According to the facility operator, an additional (6) roll-off containers will be removed from the location and taken to Austin Masters. There was a Complete Energy Service Inc. (CES) brine truck on location with the proper UIC number displayed (#25-423). An empty roll-off container has been placed on secondary containment to continue cleaning the offloading pit.
1518794987 2018-02-16 Salt Water Injection Well No Violations I was on location to check on the progress for the removal of the roll-off containers. I found six (6) containers have been removed: Three (3) by Ground Tech and three (3) by Wolford. According to the invoice, all six (6) of the containers have been taken to Austin Masters. There is a NAJ Trucking on location with a bulk trailer full of lime to be added to four (4) roll-offs for solidification prior to hauling them to Austin Masters.
1519397059 2018-02-23 Salt Water Injection Well No Violations I was on location to check on the progress for the removal of the roll-off containers. I found 32 of the roll-off containers have been removed. There are two (2) roll-off containers and two (2) vac boxes still left to be removed. PAC has scrapped up the impacted material from under the containers on the east side of the site and replaced with clean soil and gravel. The impacted material was placed in a roll-off container for proper disposal with Austin Masters. A Spirit Services brine truck arrived on location with the proper UIC number displayed (#25-562).

1535114606 2018-08-24 Salt Water Injection Well No Violations I found Odyssey on location to clean the sludge out the Off-loading pit. There are nine (9) vac tanks and one (1) half-round container with a tarp. All of the containers are empty. The tanks and container are on the concrete pads. The facility operator, Steve Yendrich, was removing the brine fluid from the Off--board space throughout the Off-loading pit. I observed no spills or leaks on site. There were no brine trucks off-loading during my inspection. According to Mr. Yendrich, the containers will be removed as they become full.
1536682203 2018-09-10 Salt Water Injection Well No Violations I found two (2) Smith Transport brine trucks on location. My initial inspection showed Smith Transport brine truck #VB-045 has no UIC number displayed. The truck was empty. The other Smith Transport brine truck #VB-029 had two (2) UIC numbers displayed on removeable placards. I questioned the driver without the UIC number. The driver for #VB-045 stated he barrowed one of the removeable placards from the driver for #VB-029 prior to hauling the brine from Pennsylvania to PAC facility. Once on location, the driver for #VB-045 stated he returned the removeable placard to the other brine truck after his truck was emptied. After reviewing the active and suspended brine haulers list, I found Smith Transport was on the suspended list for Insurance and 2017 Annual Report. I spoke with ODNR UIC Dominique Haneberg-Diggs. I spoke with facility operator and instructed him to cease all injection operations until the pressure at the injection well has dropped below the maximum allowable pressure. The facility operator stated the injection well has been shut down all morning.
