|  Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 100 pounds pressure and the 9.625 casing well head reads 1,000 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.

| On this day I witnessed the displacement of the bottom section plug below the DV tool which is set at 4710'. The plug was displaced with 154 barrels of oil based drilling mud on top of the wiper plug and chased with 340 barrels of KCL. The 2 stage relief tool was dropped and allowed to fall to the seat for a time frame of 30 minutes. Upon the opening of the tool relief ports the hole shall be circulated with KCL for 8 hours by the rig mud pumps allowing the bottom section cement to cure. The top section shall then be cemented.

There are (6) producing wells, (6) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 60 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 80 psi.

1261280386 2018-03-07 Production Wells No Violations Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads 100 pounds pressure and the 9.625 casing well head reads 1,000 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.
1371296683 2013-06-14 Casing String Witnessed No Violations Witness cementing of casing string Express Energy Services drilled to 90 of 32 hole and ran 90 of new 26 casing and cemented it in place with 500 sacks cement with 45 bbl of good returns.

Note: Driller encountered water 8 below ground level and also they encountered extremely hard drilling@85 they called and we granted them modified setting depth of conductor pipe.

1372266443 2013-06-26 Casing String Witnessed No Violations On 06/25 & 26 I witnessed the running and setting of 392 of 18.625 surface casing with 100% cement excess ran and 75 barrels of good cement returns to surface. The casing was pressure tested to 700 psi which was 500 over lift pressure with no recorded pressure loss. The mix water was tested and showed no chlorides prior to cementing. We had several job delays due to lightening.

1375214600 2013-07-30 Casing String Witnessed No Violations Status check inspection of the running of the 5.5 production casing, 17,685 of production casing has been ran and set in the well bore with proper centralizer placement along with a casing shoe and float collar ran. The well bore had over 2500 units of 3rd party gas and the company was circulating oil based mud trying to kill the gas pressure in order to start the cementing process.

1380728731 2013-10-02 Completion Testing No Violations Inspection of the completion process found the wire line company running in the hole to set a plug at 14,571 and perforate the 16th stage from 14,555 to 14,389 for a total stage length of 197. A status check on the preliminary restoration found that vegetation has been established and erosion and sedimentation control is in place. With a tank production fluid tank battery erected on site with metal containment.
1383149360 2013-10-30 Production Wells No Violations The well is the 3rd well on the drill pad to be completed and is now capable of production, There are 8 400 barrel production tanks on location in use.
1429716572 2013-07-16 Casing String Witnessed No Violations On this day I witnessed the displacement of the bottom section plug below the DV tool which is set at 4710. The plug was displaced with 154 barrels of oil based drilling mud on top of the wiper plug and chased with 340 barrels of KCL. The 2 stage relief tool was dropped and allowed to fall to the seat for a time frame of 30 minutes. Upon the opening of the tool relief ports the hole shall be circulated with KCL for 8 hours by the rig mud pumps allowing the bottom section cement to cure. The top section shall then be cemented.
1562463184 2019-01-29 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (6) producing wells, (6) gas processing units, (4) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 60 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 80 psi.
1610460774 2013-07-08 Drill / Deepen / Reopen No Violations Status check inspection plug was set at 4300 over a lost 12.250 bit with the top of the plug at 4300. The company is currently drilling at 3616 starting their 6 degree deviation kick around the lost bit and should fall back in at 4825 under the bit. A cement ticket was received from the kick plug.
1852827231 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the back of the pad, and a midstream facility behind the gas processing units.

2041437847 2013-09-24 Completion Testing No Violations Inspection and status check of the completion process found the 7 stages of the well bore have been completed and Arrow well services rig is making a cleanout run on the production casing. I met with several of the service company and producing company safety representatives on site and discussed the flow back and clean out procedures, proper well head control is in place and the clean out is to be performed with fresh water with no produced water to be reused eliminating the chance of condensate being introduced to any pump on location.
