1250191471 2009-08-11 CASE 09-003

The Columbus office received a complaint from Ms. Connie Bejger at 8553 Werger Road in Windham Township, Portage County. The well involved is the L&P #1 operated by Equity Oil & Gas. The complaint involved the onsite disposal of vines and branches that Equity removed from the tank battery and ruts that were created when Equity worked on the separator. This inspection found no evidence of debris piles in the woods west of the tank battery and the complaintant is mowing next to the containment dike around the tank battery and near the separator. The only issue is the height of vegetation inside the tank battery. There are no violations at this time.

1964385146 2004-02-04 CASE 04-004 I received a call from the complaintant that the gas leak near the Pascizynk tank battery had returned. An area four feet across 20 feet due south of the tank battery had gas bubbling to surface. There was no sheen. I contacted Everflow who again stated that the gas line inquestion was not theirs. The next well west of the Pascizynk is the L&P #1 operated by Equity Oil & Gas. I contacted Equity who confirmed that the sales line was theirs. I met on site with Don Toth who stated that Wade Pol Contracting would be on site the next day to repair the leak. I informed the complaintant.
