1409320033 2014-08-29 Witnessed 30 sacks of Class A cement set through tubing from 4900-4505 feet. Pulling tubing today.
1409678054 2014-09-02 Casing ripped at 3925 and pulled to 3753 to set Rip/Lime plug in conjuction from 3750-3503. Berea Plug set from 1817-1580 with a bag of LCM, after pumping the hole was on vacuum. Asing was pulled to ensure there was no cement in the pipe. Another 200 feet of cement was placed in the hole with the intention to shut off the Lost Circulation Zone.
1409753361 2014-09-03 Operator ran 4.5 down and tagged last plug at 1752'. They then pumped on the hole and broke circulation. Once the hole was clean of oil, they set another plug across the Berea from 1586-1735'. Operator then set the bottom of the surface casing plug from 1177-1377'. They pulled all casing from the hole and set the surface plug (0-200'). The cement stayed at the surface.

1920016624 2014-08-28 Chipco, LLC was on location running the tubing back down into the hole to 4900'. They will set bottom hole plug and rip the 4.5 tomorrow morning.
1948768261 2013-08-30 Compliance Action Follow-up -6280494: The valves were open at the wellhead, however the same old chart was on the meter. I called Tim with Enervest and asked if the well capable of production or in production and he said he would check and get back to me on the issue. Enervest has been reporting zero production. Tim had called back and stated well was in fact idle.
-440556692 2013-12-20 Compliance Action Follow-up -6280494: There has been no resolution to any of the issues. The well is currently not in production.
-6280494 2013-06-07 Well was found to be shut in at time of inspection and production has been reported as zero since 2007. There was an old chart in the meter in which the ink had bled through the paper. There is one 100 bbl tank and one full 50 bbl steel tank in the dike. Operator must make well capable of production within 60 days; or plugging is required in 60 days.
