2629460 Primary Facility 2017-08-23 Routine/Partial Inspection No Violations Noted At the time of inspection, Patterson 259 is onsite on the 3H well. This well had production casing cemented using a lead 1 of 250 sxs (53 bbls) HalCem, 14.5 ppg, 1.2 yield, a lead 2 of 760 sxs (162 bbls) ShaleCem, 14.5 ppg, 1.2 yield and tail of 650 sxs (211 bbls) SoluCem, 15.2 ppg, 1.825 yield. Displaced with 229 bbls and bumped plug. Floats held (3.5 bbls back). 80 bbls of cement returned to surface. A float shoe and float collar were used along with 186 centralizers.
2629532 Primary Facility 2017-08-15 Drilling/Alteration No Violations Noted
2645087 Primary Facility 2017-10-10 Administrative/File Review No Violations Noted
2668283 Primary Facility 2017-12-07 Routine/Partial Inspection No Violations Noted On site for a routine partial inspection.

Post stimulation tear down and activity in progress.

All temporary storage tanks, fuel storage, and portable generators, etc on containment. Well development pipeline being removed. Pad liner in place and intact. AST installed with containment.

Gathering pipeline work continues. Pipeline header installed. E/S controls in place, intact, and functioning. Sound wall installed. Site has good vegetation growth.

2670149 Primary Facility 2017-12-04 Drilling/Alteration No Violations Noted At the time of inspection, Universal Well Services was onsite stimulating well. Notification was given that stage 17 of job communicated with Snyder Brothers' Concordia Lutheran Church 2-679 well. The Snyder well was shut in and is around 1000' from the wellbore of this well. There were no releases with the communication and integrity of both wells remain intact. Penn Energy and Snyder Brothers met and had an agreement on pumping pressures, which would be around 70 bbls/min.

Currently, stimulating stage 18. Unsure of total stages because some are not breaking down and may be re-perfed.

2678361 Primary Facility 2018-01-04 Administrative/File Review No Violations Noted
2679437 Primary Facility 2018-01-04 Routine/Complete Inspection No Violations Noted On 1/04/2018, WQS Brian MacQuarrie and WQSS Scott Lux onsite. Meet with Frank Kocher (Penn Energy subcontractor). Sandtraps setup at three wellheads. Large AST setup onsite for impaired water. Secondary containment maintained on well site. Snow on ground. No apparent violations.
