0002849950 | 2014-11-07 | Production Wells | No Violations | My inspection today finds the company has moved in a Matheny Well Service rig to attempt to repair the well. There are no personnel onsite. The tubing has broken completely off of the well and the wellhead has been partially partially disassembled. A valve on the 4.5 casing wellhead is open and a small amount of gas is flowing out of it. |
0223774724 | 2013-11-06 | Production Wells | No Violations | My inspection today finds the well is equipped with 4.5 casing and 1.5 tubing. The tubing has a rotted off at the top of the well head. A significant amount of gas is leaking from the tubing. A production unit is present off site but is not hooked up. The well is not capable of production. |
0816818718 | 2016-03-02 | Production Wells | No Violations | My inspection today finds the well is not capable of production. No gas or oil transportation pipelines are attached to the wellhead. All of the valves at the wellhead are shut. The off site production unit is not connected to any pipelines. |
0926906401 | 2009-10-21 | Production Wells | No Violations | FOLLOW UP TO NOV # 1259784279 Inspected site along with supervisor and determined that well is idle and incapable of producing in its present state of repair,it is equipped with 8 surface casing and 4.5 production casing and 1.5 tubing which has hole in it at tubing head, the last production reported was back in 1998 and I have had several conversations with company rep requesting that they plug or produce said well. |
0939117533 | 2014-10-28 | Administrative Inspection | No Violations | I received a telephone call from John Schneider, Production Supervisor for Stonebridge Operating, indicating that the company intended to make an attempt to repair the well in the next few days. |
1097896989 | 2014-02-25 | Production Wells | No Violations | My inspection finds the well is equipped with 4.5 casing and 1.5 tubing. The tubing has a large hole in it at the top of the 4.5 by 1.5 wellhead. Gas can be heard leaking. The casing appears to have partially collapsed at the top of the 8 and is beginning to lean. A production unit is present off site. There is no tank for this well. The company reported gas production for the well in 2012, as well as 72 bbl of brine. Previous inspections have indicated the well is not capable of production. I called and left a message for John Snyder to call me to discuss this well at 12:06 pm. |

1415664647 | 2014-11-10 | Work Over Reconditioning | No Violations | My inspection today finds Matheny Well Service is attempting to utilize a tubing spear to pull the 1.5 tubing out of the wellhead. The Stonebridge representative, John Schneider, stated that he believed the tubing has crimped a little at the wellhead slips and is preventing the tubing spear from entering the tubing enough to pull it. Another attempt will be made on Wednesday. |
1434987604 | 2015-06-22 | Production Wells | No Violations | My inspection today finds the well is not capable of production. There are no gas or oil transportation pipelines attached to the wellhead. All of the valves at the wellhead are shut. The off site production unit is not connected to any pipelines. No tank associated with this well can be found. |

1495488114 | 2017-05-22 | Plug / Plug Back | No Violations | The purpose of my inspection is to witness plug back operations. The well prior to plug back operations was a total depth of 5650 with 8.625 inch diameter surface casing at 532 feet, 4.5 inch diameter production casing at a depth of 5650 feet, 2.875 inch diameter intermediate set on a packer at 30 feet, and 1.5 inch diameter tubing 4554 feet deep. The new proposed total depth is 3929 feet and the plans are to produce from the Devonian Shale formation. Huffman Bowers is on location with their service rig, pump truck, water truck and cement truck. The 1.5 inch diameter tubing is stuck within the 4.5 inch diameter production casing. The service rig tagged the inside of the 1.5 inch diameter tubing at 4800 feet in depth. Jet Wireline rigged up and ran down to 4604 feet and attempted to shoot the tubing free. The first and second attempt was unsuccessful shots did not fire. The shots were fired at 4604 feet on the third attempt but the wireline became stuck down hole. The wireline was retrieved but the collar locator was not retrievable. |
1496928583 | 2017-06-08 | Final Restoration | No Violations | The purpose of my inspection was a status check on a final restoration. During my inspection the well location and the lease road had been restored. The pit used in plugging operations had been closed. The area surrounding the plugged well and the lease road had been graded, seeded, and mulched. Vegetation had been established. All production equipment and debris from plugging operations had been removed. The location meets 1509.072 (B) Final Restoration requirements. |
1506484810 | 2014-09-22 | Production Wells | No Violations | My inspection today finds the status of the well has not changed. The well is still incapable of production. The 1.5 tubing has a large hole in it and the entire well is leaning. Gas is still escaping from the hole in the tubing. |
1834179882 | 2013-07-11 | Administrative Inspection | No Violations | I called and spoke to Mr. John Snyder. I told him that the plug back permit for the well would be expiring soon and that something needed to be done with the well. Mr. Snyder stated that he intended to begin work on the well in the next two months weather permitting. |
1877776063 | 2014-11-12 | Production Wells | No Violations | My inspection today finds the service rig crew has successfully used a spear to pull the tubing enough to strip the 4.5 by 1.5 tubing wellhead off. 2.875 tubing with a packer was placed over the 1.5 tubing. The packer was set and the tubing was then packed off in a wellhead on the 2.875 tubing string. The service rig was unable pull the tubing. John Schneider of Stonebridge Operating believes the tubing is stuck in the well at considerable depth. |

The service rig pulled the 4.5 inch diameter casing up 50 feet from the rip at 1920 feet and set a 100 foot isolation plug with class A cement within the open 7.875 inch diameter open hole from 1870 feet to 1770 feet. The 4.5 inch casing was pulled up to 1642 feet and the Berea plug was set within the 7.875 inch diameter open hole from 1642 feet to 1490 feet with class A cement. The 4.5 inch casing was pulled up to 611 feet to set the bottom of surface casing plug from 611 feet to 411 feet in depth with class A cement. The 4.5 inch diameter casing was removed from the hole and a 150 foot surface plug was set using class A cement. Cement stayed at the surface. A compliance notice issued for this well in 2009 states violations 1501: 9-9-05(A)(10) Failure to legibly identify well and 1509:12B Well shall be plugged. The plugging of this well resolves all violations from compliance notice 1259784279. |
