The well was acidized following drill out of the sand and cement plug placed on bottom to facilitate casing repairs. The operator believes some of the cement made it to the formation. The well went on vaccuum following the job. No break was observed.

0009294602 2016-01-28 Administrative Inspection No Violations A conference call was conducted regarding gas pressure on the intermediate casing. Bob Worstall, Will Ziegler, Steve Opritza, Dave Claus and Tom Benko of ODNR participated. Current well status was discussed. Inspection of the well prior to the call revealed that the tubing and production casing are shut in. Gauges on the tubing and production casing indicated 900 psi and 400 psi respectively. A gauge on the flowline indicated 150 psi. The intermediate casing is valved to the line and is open to it. 50 psi was indicated on the outlet side of the heater treater. The operator indicated they are still testing the well and attempting to determine what course of action they will take. The well was not acidized. The 4.5 inch casing is cemented to 3850 feet. Deputy Chief Worstall requested daily pressure monitoring of the casing strings including surface casing. The operator stated they have no wellhead on the surface casing so monitoring that is not possible. The surface casing annulus was filled with cement when the intermediate string was cemented. The pumper was asked to submit daily pressures to the inspector. Jim Aslanides of MFC contacted the inspector and stated they would not do so. Mr. Aslanides requested Deputy Chief Worstalls phone number to contact him directly. MFC assured the inspector that they are keeping pressure on the 8 inch casing at a safe level. The welltender stated that the casing blows down to line pressure in a few minutes and that it has only pressured up to 350 to 400 pounds at most. Its not clear if gas is coming from the Clinton or Lime formations or if there is a leak in the 4.5 inch production casing. MFC is able to kill the well if necessary to reduce gas pressure on the intermediate casing. MFC is going to bring a service rig on the well by mid February to attempt to determine if they have a leak on the production casing and make repairs if necessary.

They will either be replacing the damaged joint or removing the collar from the casing still in the well and installing a casing patch.

0559040714 2015-12-23 Production Wells No Violations Inspected well and met with operator on location as a result of an article in the Ashland Times-Gazette newspaper. The valve on the 8 inch intermediate casing was open and gas odor was observed. The Nankin Fire Department responded to a call at 10:40 am on December 22, 2015. The road was closed temporarily until the valve was closed and the site was released. No residences were evacuated during the event. The operator hired a pump truck to load the 4.5 inch production casing with 9 pound water and conduct tests on the well. Initially the 4.5 inch casing had 1750 psi; the well was loaded with 70 bbl of water, after 30 minutes the well was building pressure. An additional 25 bbl of water was loaded into the well and the well went onto a vacuum and had been killed. Upon arrival at 12:50 pm the well was on a vacuum for more than one hour. The location continued to be under control. The valve in question is normally maintained in the closed position. The well has not been put into production.

They advised that the landowner reported them for trespassing and the sheriff was called. The original lessor passed away and his heirs are unhappy with the lease. MFC provided lease information to the county prosecutor to prove it was not a law enforcement matter. The sheriff stated that they will no longer be involved if further complaints are received.

Jim Aslanides of MFC stated they shot their drill bit off the tubing on Friday, 5/6/16. The bit became stuck when drilling out the formation packer to complete the well. A plug consisting of 25 sacks of sand was placed on bottom. The sand was followed by 40 sacks of Halliburton Halcem cement at 15.8 pounds per gallon, 1.18 cubic feet per sack. Halliburton states the plug is at 4482 feet. The operator intends to make repair to the production casing. MFC advised that they would be working on the well but they did not notify that they would be placing a sand and cement plug on bottom. Per Deputy Chief Worstall, this does not warrant a compliance notice.

The casing leaked during the test. The leak is at the same location as before. Appalachian Well Survey was re inspecting the casing with a camera. They believe the collar that was left in the hole may be bad also. They are going to attempt cutting the collar and installing a casing patch.
0988039975 2015-07-09 Drill / Deepen / Reopen No Violations The operator is drilling on air and fluid at 860 feet. The bit is stuck and circulation has been lost. Poulson Drilling is attempting to free the bit. MFC may decide to skid the rig. Followup in the afternoon revealed that the rig had been unable to free the bit. They are planning to shoot off the bit in the morning and attempt to recover it with an overshot tool.

They rigged up an overshot tool consisting of 8.625 inch casing and washed over the bit with heavy drill mud. No obstruction was encountered at the red rock. The bit was trapped by cuttings from an improperly conditioned hole caused by aerating the drilling fluid. They were able to hook onto the bit with drill pipe following cleanout of the hole.

Jim Aslanides of MFC Drilling stated in a phone conversation on 4/12/16 that the intermediate casing only pressures to a maximum of 20 to 25 psi and they are blowing it down every other day. He also stated the tubing and production casing have no pressure on them and the trempealeau formation is killed. They are coordinating with Halliburton to come and service the well. He does not have a timetable at this time.

1497744899 2016-05-20 Drill / Deepen / Reopen No Violations Appalachian Well Survey ran a wireline camera to inspect the production casing. They were unable to get the camera past an obstruction at 1600 feet. There was some type of anomaly observed between 1500 and 1600 feet but they were not able to determine if there is damage to the casing pipe at that depth or if some other obstruction such as cement is at that depth. They are speculating that it may be a hole but they need to do further analysis before proceeding with a repair. The well is killed and there is a sand and cement plug on bottom. There is no gas leakage from the Trempealeau formation. The operator ran 2 year old used pipe in the well and provided information that it was tested and drifted. They believe the casing may have been damaged by handling or when it was installed in the well. The pipe was not observed when it was ran in the well.

The plan is to pump water through the tubing below the packer to pressurize the 4.5 inch in order to locate the hole. The rig will pull the packer up and repeat testing until the hole is located by loss of pressure. The well has been sealed by the bottom plug placed by Halliburton. No gas pressure was on the intermediate-production annulus when the rig crew checked it.
