0184388022 2015-08-03 Production Wells Violations Noted My inspection of this well found the 4.5 production casing has a swage to two inch with a two inch valve which appears to be partially open. The well was not connected to anything. There was no visible flow line near the well location. The surface casing head was above the ground level. A two inch valve on the surface casing head was open. There was no identification posted at the well location. The tank battery for this well is unknown at this time. The company has not submitted production reports for this well for 2014. The company reported 0 production for this well from 2001-2012 and reported 801 MCF of gas and 24 barrels of brine of production in 2013. This well was incapable for production at the time of my inspection. REMEDIAL WORK REQUIRED: Establish identification at the well location, submit production records for 2014. Big Sky Energy Inc. shall submit a written plugging schedule to Jerry Kohl at The Ohio Department of Natural Resources, Division of Oil and Gas Resources Management, 3575 Forest Lake Dr. Suite 150, Uniontown, Oh. 44685-8116 within 30 days, per OAC 1501:9-11-05 [ B ], and plug the well within 180 days. Big Sky Energy Inc. will be notified if the plugging schedule is approved or denied.
0646152580 2016-09-13 Production Wells No Violations My inspection found the status of the well has not changed. There was no identification posted at the well location. The well has not been plugged.
1312510761 2018-03-06 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 184388022. At the time of this inspection, the well had a 4.5 inch casing swaged into a 2 inch line. The valve on the well was cracked open. There was no signage at the well location and no associated production equipment. Signage still needs added to the well location and a plugging plan still needs submitted.

1783840323 2017-09-21 Production Wells No Violations I inspected this producing well as a follow-up to a compliance notice that had been issued directing the owner to complete the following: properly identify the well with compliant signage, submit missing annual reports of production, and submit a plan to plug and abandon the well. At the time of this inspection I observed the following: a wellhead with 4.5-inch to 2-inch reducer swage with Demco valve that was closed. There was no other production equipment present or active connections. There remained no identification signage, no annual production reports filed since 2014, and no evidence that attempts have been made to plug the well.
