0536991050 2019-04-08 Production Wells No Violations I conducted a compliance notice follow---inch casing swage. The production valve was open at well. The tank battery consisted of two 100- barrel production tanks, mechanical separator, and one-meter run with a chart. The load line to the tanks production valve was secured with a plug. An earthen dike surrounded the tank battery. Proper identification was posted at the tank battery. The lease road was in stable condition. Noted in last report that flow line was disconnected. Since that time repairs have been done and well has been put back into production.

1380745877 2018-11-07 Production Wells Violations Noted I conducted an inspection on this well. At the time of this inspection, the well consisted of a 4.5 inch casing swaged into a 2 inch line. The 2 inch line from the well was disconnected from the flow line, rendering the well incapable of production. The valve on the casing swage was closed. The last production submitted that met state requirements was in 2014. The tank battery was comprised of two 100 BBL tanks and a mechanical separator. The valves on the separator were closed and no chart was present in the box on the separator. The diked tank battery had legible signage. The lease access was stable. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
