0056189846 2017-11-02 Drill / Deepen / Reopen No Violations U S Energy Rig # 9 spudded the well on 1/1/2017 @ 1600 hrs. and drove 20 drive pipe to 25 KB. Brown sandstone bedrock was encountered at 21 KB. A 17.5 borehole was drilled on fresh water to 72. Two (2) joints of 13.635, 48 lbs. / ft., ST&C casing were lowered to 62. Petroset Cementing Services established circulation, mixed a cement slurry and circulated the slurry back to the cellar. The cement load was 100 sacks of Class A blended with 3 % Ca Cl2 and 40 lbs. of cottonseed hulls. Approximately eight (8) barrels of cement returns were observed to the cellar. Mix water temperature 42 F and pH was 7. Cement level in the 20 X 13.375 annular space was 12 down and static one (1) hour down after displacement. No violations were observed during this inspection.
0338468773 2018-05-15 Preliminary Restoration No Violations The access road was graveled to the pad. The pad covered an area approximately 180 X 200 and contained two (2) well, the Mate Unit # 2-H and the Mate Unit # 1-H ( 34-007-2-4563 ). The drilling pits were filled and rough graded, but not seeded. Cary LeShock ( U S Energy ) said, the landowner wishes to seed the disturbed area himself to prevent weed invasion. The Mate # 2-H was completed as an electric pumping well and was operating during my inspection. The tank battery was constructed on the the north side of the pad and contained three (3) 210 barrel stock tanks and one (1) Gas Production Unit (GPU). The loadlines were plugged and locked and an earthen dike surrounded the equipment. The dike was seeded and covered with straw, but not growing at the time of inspection. Proper spacing was observed between the equipment. The I. D. information was handwritten on the 210 barrel tanks and pumping units. A three (3) inch plastic gas sales line was awaiting connection to the sales point. The well was started pumping on 5/9/2018. Preliminary Restoration conditions have not been met and a re-inspection is warranted.

I reviewed the casing program and Inspector call points with Cub ( U S Energy tool pusher ). Access to the location was graveled. No violation identified during this inspection.
1043792769 2017-11-17 Drill / Deepen / Reopen No Violations U S Energy Drilling Rig # 9 was drilling on saltwater based drilling mud at 4658 MD. The lease road and pad were graveled and accessible. The drilling pits, both cutting and circulation, exhibited adequate free board and volume. The cellar was liquid tight. The mud pump area was free of leaks and any contamination. No violations identified during this drilling inspection.
1168733386 2017-11-09 Casing String Witnessed No Violations U S Energy Drilling Rig 9 drilled a 8.75 borehole to 3339 KB. The drilling rig ran 80 joints of 7, 20 lbs. / ft. casing to 3314 KB. Petroset Cementing Services mixed a 84 barrel cement slurry consisting of Class A cement blended with 3% CaCl. Displacement was achieved with a wiper plug and 134 barrels of fresh water. Mix water temperature was 39 F and pH was 7. The density of the cement slurry was verified, by scale, at 15.2 ppg. The wiper plug was landed at 500 psi and increased to 1000 psi for five (5) minutes without any pressure decline. The pressure was released with .5 bbl. flowing back and stopping. The float shoe held. Circulation was maintained throughout the entire cement operation.
1510254460 2017-11-09 Drill / Deepen / Reopen No Violations At the time of my inspection the operator had drilled to a depth of 3,339 feet, the hole diameter was 8.75 inchs. During my inspection the operator removed the BOP stack out from under the rig floor and start running the 7 inch intermediate casing into the hole. The bottom joint of 7 had a shoe on it. Seven inch bow spring centralizers where on site. Visual observations of the hole indicated static conditions. A pallet of solid centralizers were also on site for the production string. Pits were in good condition. Access road was stable. Drilling pad was in good repair. Permit was posted in the dog house.
1893609010 2018-03-08 Drill / Deepen / Reopen No Violations Moore Well Svc on location drilling out various plugs. All plugs drilled out today and crew is building well head to start swabbing well before running rods. ID signs are posted. No noted violations noted.
1900226089 2017-11-22 Casing String Witnessed No Violations U S Energy Rig #9 drilled 6.25 borehole to 5640 MD. The rig ran 136 joints of 4.5 to 5617 MD. Petroset Cementing Services mixed and displaced a 84 barrel cement slurry consisting of Class A cement blended with 3% CaCl. Mix water temperature was 39 F and pH was 6. Cement slurry density was verified, by scale, at 15.4 ppg. A five (5) wiper latch down plug displaced the cement slurry using 87 barrels of fresh water. The plug was landed at 600 psi and increased to 1200 psi for five (5) minutes. The green cement test was passed. Circulation was maintained throughout the job. No violations identified at this inspection.
