0179155431 2018-02-09 Drill / Deepen / Reopen No Violations U S Energy Rig # 9 was rigging up on the Kuhar pad. I reviewed the casing and notification points with Cub ( toolpusher ). The pits were being pumped out due to snow melt, inorder to place the liner. No violations identified during this inspection.
0244985011 2018-02-20 Drill / Deepen / Reopen No Violations U S Energy Rig # 9 was shutdown, waiting on a replacement air booster. The drilling was halted at 2205 KB. A flow of saltwater ( 3-5 gpm, reported by driller ) was encountered at 2177. The pits had adequate freeboard. I inspected the cellar and did not see any gas migration from previous casing strings. The drilling pad was in bad shape with deep ruts due to the frost coming out of the ground. U S Energy personnel was repairing the disturbed area. No violations identified during this inspection.
0327568898 2018-05-15 Completion Testing No Violations The lease road and pad was rocked and accessible. The well was surrounded by jersey barriers. The 9.625 X 7 and 7 X 4.5 annular valves were open and free from any flow. There were 14 -500 barrel frac tanks on location in preparation for the frac job which was scheduled for 5/21/2018.The pits were closed and rough graded. There were two (2) mud pumps, one (1) substructure and a pipe rack of drill pipe left on location. The drilling equipment will be removed once the drilling rig starts. Final restoration will be scheduled once the frac and completion activities are done.
0329360183 2018-02-13 Casing String Witnessed No Violations U S Energy Rig # 9 drilled a 17.5 borehole from 10 to 65 on fresh water. After reaching total depth the borehole was conditioned by circulating fresh water for 1.5 hours. Two joints of 13.375 O.D. diameter , 36 lbs. / ft., 8rd, ST&C casing were lowered into the borehole. One (1) bow spring centralizer was attached at approximately 40. Petroset Cementing Services (PCS) was the cementing contractor. The cement load was 150 sacks of Class A cement blended with 3% CaCl. PCS connected to the 13.375 casing and established circulation and conditioned the hole by pumping 30 barrels of fresh water. PCS mixed cottonseed hulls and the cement slurry and displaced it back to surface. Mix water was 40 F and pH was 7. I saw approximately 8 barrels of cement slurry circulated back to the cellar. The cement slurry was displaced to 55 inside the casing. No violations identified during this inspection.
0622710682 2018-02-12 Drill / Deepen / Reopen No Violations U S Energy Rig # 9 drilled a 17.5 borehole to bedrock at eight (8) feet. A 12 piece of 20, 65 lbs. / ft was driven to 10 utilizing the air hammer on top of the casing with a cap welded on. The drilling permit and all conditions was posted on location.
0633296019 2018-02-13 Casing String Witnessed No Violations I was on location to witness the placement and cementing on the 13.375 casing string. Shannon Mullins was on site for a training exercise. U S Energy Rig # 9 drilled a 17.5 borehole from 9 to 69 KB on a fresh water system. After circulating the borehole for one (1) hour the drilling rig lowered 61 of 13.375, 48 lbs. / ft., ST&C casing into the borehole. One bow spring centralizer was placed on the casing at 42. Petroset Cementing Services (PCS) was the cementing contractor. PCS connected to the casing via a swage and pumped 30 barrels of fresh water to establish circulation. A cement slurry ( ~ 29 barrels of Class A blended with 3% CaCl, 40 lbs. of cottonseed hulls ) was mixed and circulated back to the cellar. Cement slurry returns were identified, in the cellar, with nine (9) barrels of displacement left. Displacement was to 55. Circulation was maintained throughout the job. The casing seat pressure test will be conducted after the cement slurry reaches sufficient compressive strength. No violations identified during this inspection.
1024036555 2018-03-26 Drill / Deepen / Reopen No Violations U S Energy Rig # 9 was drilling a 6.25 borehole at 3005 KB on misted air in the vertical section. I inspected the backyard air pack, mud pumps, pits and found them in compliance at the time of the inspection. The rocked drilling pad was loosing its stability with a large portion undriveable. The lease road continued to be stable. Repairs, to the pad, were scheduled to be made when weather permits.
1347205574 2018-02-22 Casing String Witnessed No Violations U S Energy Rig # 9 drilled a 8.75 borehole from 339 to 3005 KB on a misted air drilling system. The rig circulated the hole for approximately one (1) hour prior to removing the drill string. A casing string comprised of 70 joints of 7, 20 lbs./ft., ST&C, 8rd, R-3, 15 bow spring centralizers ( 1st collar then every other collar ) and 7 float shoe. Total tally ran was 2980 of casing. Petroset Cementing Services (PCS) was the cementing contractor. PCS connected to the plug container and loaded the hole with 30 barrels of fresh water. Mix water temperature was 37 F and pH was 6. PCS mixed a cement slurry consisting of 400 sacks ( 84 barrels ) of Class A blended with 3% CaCl . Displacement was achieved with a wiper plug and 120 barrels of water. The plug landed, at the correct displacement volume, at 600 psi and was increased to 1100 psi for five (5) minutes. After five (5) minutes without any pressure decline the pressure was released. One half (1/2 ) barrel flowed back and stopped. The float shoe held. Continuous circulation was oserved throughout the entire job.
1442891527 2018-02-15 Casing String Witnessed No Violations U S Energy Rig # 9 drilled a 12.25 borehole from 65 to 339 KB on a fresh water drilling fluid system. A casing string consisting of eight (8) joints of 9.625, 26 lbs./ft., ST&C, R-3, four (4) bow spring centralizers and 9.625 float shoe was lowered into the borehole. Petroset Cementing Services (PCS) was the cementing contractor. PCS connected the casing via a plug container and pumped five barrels of fresh water to load the hole. A ten (10) barrel spacer of gel (200 lbs.) and cottonseed hulls ( 40 lbs. ) was pumped prior to the cement slurry. Mix water temperature was 38 F and pH was 7. Approximately 32 barrels of cement slurry ( ~ 150 sacks Class A blended w/ 3% CaCl ) was mixed and displaced utilizing a wiper plug and ~26 barrels of fresh water. The cement slurry weight was verified, by scale, at 15.1 ppg. The plug was landed at 200 psi and bumped to 700 psi, but a leak on the plug container prevented an accurate test. I requested the driller to pressure test the casing in conjunction with the Blow Out Preventer (BOP) test, chart the results and forward for review before drilling forward. I witnessed approximately 10 barrels of cement slurry returned to the cellar. Steve Tompkins and Shannon Mullins were on location for a training exercise. No violations identified during this inspection.
1518722528 2018-02-15 Drill / Deepen / Reopen No Violations I was at this location to witness activities associated with surface casing installation and cementing. Also on site during these activities were Inspectors Bob Roush and Shannon Mullins. At the time of this site visit, crews from the drilling contractor (US Energy) had reached the target depth for surface casing. Drill collar was removed and 9.625-inch surface casing was advanced. Circulation was established with no observed losses of fluids, and no observed gas production. Cement mix water was evaluated prior to cementing and found to be 35 degrees (F), and pH 6. Cement was mixed and displaced with good returns. Following latch-down, it was noted that the pressure could not be maintained steady. The source of the pressure loss was not apparent. It was agreed that the surface plug would be pressure-tested following curing in 8 hours.

The drilling contractor is US Energy. During my inspection, the following activities occurred: surface casing was installed and cemented in place, with good cement returns observed at surface.
1519666202 2018-02-26 Drill / Deepen / Reopen No Violations U S Energy Rig # 9 was conditioning the borehole at 3005 KB prior to removing the drill pipe to change the bottom hole assembly to allow directional drilling. I inspected the pits, air pack and cellar and found all were in compliance. No violations were identified during this inspection.
1520598561 2018-03-08 Drill / Deepen / Reopen No Violations U.S. Energy Rig #9 drilled a 6.25 well bore to 5694 and ran a total of 5665.5 of 4.5 / 11.6 lb./ft. production casing with 24 centralizers and a landing baffle at 5659. Petroset Cementing Service established circulation with 25 barrels of fresh water and pumped 400 sacks of class A cement 1.18 yld. And displaced with 88 barrels of fresh water. Plug landed 1500 psi. Plug held after 5 minutes shut in. Drilling Permit posted at rig.
1538072511 2018-09-27 Preliminary Restoration No Violations The lease road and pad was gravel and access was good. The well was completed as a electric pumping well. The seven (7) inch casing was equipped with a pop off valve and gauge. The gauge did not indicate any pressure. The well was not pumping during this inspection. The tank battery was shared with the Kuhar # 2-H ( 34-007-2-4566 ). The tank battery consisted of three (3) 210 barrel tanks. Two (2) of the 210 barrel tanks were for crude oil storage and the third was for the storage of brine. The tank battery was surrounded with a earthen dike. The load lines were plugged and locked. A Gas Production Unit (GPU) was utilized for separation of the gas and fluids. The location was rocked and the perimeter was graded, but had some areas where the vegetation had not taken hold. Preliminary Restoration could not be passed, at this time, due to the barren areas.

Keith Moore said, that circulation was lost between the third (3rd) and fourth (4th) plug. The crew was waiting on orders as to how to proceed. No violations were identified during this inspection.
