1490030545 2017-03-02 Orphan No Violations Orphan well complaint investigation: Landowner Carol Beale notified the Division of a potential orphan well on her property. Ms. Beale believes the well was plugged in the past and that the plugs may have failed. Upon inspection, it is observed that there is a fluid-filled depression located very near the coordinates listed in RBDMS for this well. The depression is approximately 24 inches across and is circular in shape. No casing is visible at the surface, however plugging records state that 300 of 8.25-inch diameter casing was left in the well at the time of plugging. The plugging records also indicate that the well was plugged with prepared clay and bridge plugs at intervals within the well bore. There is no surface equipment on the well, however a cylindrical gas volume tank is still present beside the well bore. There are the remnants of an old earthen pit below the well location which may have been the tank location, the brine disposal pit, or the original drilling pit. Ms. Beale has expressed interest in keeping the pit in place after plugging since it now acts as a vernal pool for wildlife on the property. According to Ms. Beale, there are no overhead or buried utilities to contend with to access and/or plug the well. The existing access road is in adequate condition and minimal tree trimming will be needed. Several smaller trees will need to be cut around the well location for a rig to access the well. Photos were taken of the location and access road. Ms. Beale agreed to sign and fill out a right of entry form and has supplied a copy of the quit-claim deed for the parcel. The Site Evaluation Matrix score for the well is 179.
