0209388875 2013-03-11 Production Wells No Violations Follow up to 1362601654. Most of the contaminated materials and soils had been removed from the site of the line break and around the well head. Absorbent pads and material had been laid down. The water in the settling filter was field tested for Chlorides with a low range Quan-tab. A reading of .2 was noted equal to less than .005% NaCl.

0534551987 2013-04-08 Production Wells No Violations Follow up to 1362601654. The entire area around the cut line and the ditch has been remediated. The dike has been emptied of excess water, the cut in the wall has been repaired, and the leaky tank is no longer connected. The line has been buried in the landowners yard. The soil around the area at the well head is still contaminated. I spoke with the field supervisor at the well site and informed him the area around the well still need addressed.

1347210019 2019-02-19 Production Wells No Violations I received a call from well owner Mr. Mark Hines on 1.18.19 at 7:23 AM. Mr. Hines told me he had a oil leak in the flow line that is connected to the 100 barrel storage tank. The plastic line had what appeared to be a bullet hole. The loss was approximately 1 barrel of crude oil. The oil ran down slope and stopped before entering into the roadside ditch. When I arrived there was residual oil remaining on the surface of the ground. I instructed Mr. Hines to remove the small amount of crude oil that remained. I ask Mr. Hines to contact myself when the remaining oil was removed.
1362601654 2013-03-05 Production Wells Violations Noted Dike had roughly 6 of water in it at time of inspection. Water was field tested and found to have 1603 ppm Chloride content. The dike had a cut in the side of it and was releasing a small amount of fluids onto the lease road. No contamination was noted beyond the lease road. One of the two 100 barrel steel tanks was missing plugs in both drains. The stuffing box at the well head had leaked in the past and an area 3 x 3 had become contaminated with oil. A 1 black plastic flow line running from the well to the tank had been severed in the past and released oil onto the surface of the ground. A trench had been dug to burry the line, repairs had been made to stop the leak and straw had been placed on the surface but oil and contaminated soils remained at the surface. Oil had spread over the surface at the site of the break contaminating an area of roughly 10 x 10 then moved through the trench down hill towards the road roughly 175 feet. Dike shall be emptied of fluids and the cut in the dike shall be repaired, or dike shall be replaced. Any tanks in the dike shall be equipped with locks or plugs. Stuffing box shall be repaired or replaced. All lines used in the production of oil and gas shall be buried at least 24 below the surface. All oil, contaminated soils, and oily materials shall be removed and disposed of properly. Steps shall be taken in order to encourage vegetative cover in the affected areas. Any affected areas shall have top soil replaced where necessary and be reseeded and mulched.

1752574468 2013-03-08 Production Wells No Violations Follow up to 1362601654. Water still in dike but the cut had been partially repaired with a shovel. No plugs in the tanks. There is still contaminated materials on the surface. After speaking with the landowner she pointed out to me the settling tank for there developed spring was less than 40 from the contaminated soils. The developed spring consisted of a 15 long piece of schedule 40 PVC that level inside the settling tank was less than a foot below the surface. From there the water flowed into a cistern near the house. After looking in the settling tank I did not see any immediately visible contamination. Pictures taken. Settling tank location = 39.43208 -81.91226. I immediately called Kyle Baldwin to arrange an inspection with our hydro geologist. I notified the company of the proximity to the families water supply and the need to clean the site immediately.
1763140509 2007-02-21 Production Wells Violations Noted Oil was found on the lease road, which had flowed out a gap in the tank battery dike. The small amount of oil (2-3 gallons) outside the dike was close to the road ditch. Notified Chip Smith by telephone, that the tank battery required immediate clean up and the dike needed repaired.

The leaking tank has been disconnected and is empty. The dike has been patched. The line has been partially buried, through the area that is the landowners yard.

There are still absorbent pads in the ditch. The line has not been buried yet. The leaky100 barrel tank at the battery has been emptied and disconnected. There is still water in the dike but the dike is not leaking.
