Mr. Hines told me he had a oil leak in the flow line that is connected to the 100 barrel storage tank. The plastic line had what appeared to be a bullet hole. The loss was approximately 1 barrel of crude oil. The oil ran down slope and stopped before entering into the roadside ditch. When I arrived there was residual oil remaining on the surface of the ground. I instructed Mr. Hines to remove the small amount of crude oil that remained. I ask Mr. Hines to contact myself when the remaining oil was removed. |

The dike had a cut in the side of it and was releasing a small amount of fluids onto the lease road. No contamination was noted beyond the lease road. One of the two 100 barrel steel tanks was missing plugs in both drains. The stuffing box at the well head had leaked in the past and an area 3 x 3 had become contaminated with oil. A 1 black plastic flow line running from the well to the tank had been severed in the past and released oil onto the surface of the ground. A trench had been dug to burry the line, repairs had been made to stop the leak and straw had been placed on the surface but oil and contaminated soils remained at the surface. Oil had spread over the surface at the site of the break contaminating an area of roughly 10 x 10 then moved through the trench down hill towards the road roughly 175 feet. Dike shall be emptied of fluids and the cut in the dike shall be repaired, or dike shall be replaced. Any tanks in the dike shall be equipped with locks or plugs. Stuffing box shall be repaired or replaced. All lines used in the production of oil and gas shall be buried at least 24 below the surface. All oil, contaminated soils, and oily materials shall be removed and disposed of properly. Steps shall be taken in order to encourage vegetative cover in the affected areas. Any affected areas shall have top soil replaced where necessary and be reseeded and mulched. |

Settling tank location = 39.43208 -81.91226. I immediately called Kyle Baldwin to arrange an inspection with our hydro geologist. I notified the company of the proximity to the families water supply and the need to clean the site immediately. |
