0165293016 | 2010-10-12 | Production Wells | No Violations | Inspection this day found well with jack, trees and brush have recently been trimmed around the well, appears to be producing because valves are open, no motor. Tank or well have no IDs, good dike. Talked to renter in house and he said well belonged to landowner Jim Wachtel at 740-448-7352 or Mark Cohen at 740-448-2044. Will pass along this information to VIRCO. |

1162307102 | 2014-09-24 | Production Wells | Violations Noted | Well equipped with a pumping unit with no power supply, the pumping unit was not connected to the sucker rod, and the unit was overgrown with vegetation. The valve on the production string and the tubing were closed to sales. There was a small leak in a hammer union that connects to tubing to the sales line. There was a 20 x 3 are around the well head of contaminated soils. There were no production tanks and the well was not identified. No production has ever been reported to the division for the well. Well shall be plugged. Contaminated soils shall be removed and disposed of properly, any affected areas shall have top soil replaced and steps shall be taken to encourage vegetative cover. Any leaking equipment shall be repaired or replaced. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. |
1168655071 | 2016-10-28 | Final Restoration | No Violations | On my inspection I found that the location had been restored according to 1509.072. Just waiting on the land owner waiver to be filled out for the final to be passed. The pumping unit and the tank is still on location. When I sounded the tank the tank sounded empty. |

1380928597 | 2016-06-29 | Plug / Plug Back | No Violations | I witnessed the tubing going back into the hole this morning. Cemented the bottom of the hole with 23 sacks of cement with hulls from 1460 to 1160. An older gentleman stopped by and said that they shot this well about 700 near the injun sand. I had triad set a 200 plug in the 4 1/2 from 750 to 550 through the tubing. The rest of the tubing was pulled out of the hole. Once the tubing was out it was determined that the casing head was welded onto the 4 1/2 and the 1 1/4 nipple. This was a home made casing head. Rods were ran down the 1 1/4 and mud was hit at 85. Pushing the rods down they were able to get to about 120. It was determined that perforating the pipe about 115 and getting circulation then cementing to surface was the best plan of attack. Pipe was perforated and circulation was achieved out of the 1 1/4. The cement was then ran to surface out of the back side of the 4 1/2 pipe up through the 1 1/4 pipe. |
1472160784 | 2015-12-01 | Production Wells | No Violations | Follow up to -1162307102. The well is not capable of production. The valves at the wellhead were closed and the pumping unit was not attched to the sucker rods. |
1579115038 | 2016-12-12 | Final Restoration | No Violations | I drove by the location on Thursday and saw that the pumping unit and the tank was still in place. While in the office on Monday the 12/12/16 I was handed a signed and completed land owner waiver for the pumping unit and the tank. The landowner waiver was the final criteria to pass the Final Restoration. |
