Test pressure was holding when a leak was observed on the wellhead. Annulus pressure was released. Well head was tightened by pulling tension on the tubing and tightening the wellhead one full turn. Replaced slips annulus water level stayed at the surface. Pressurized the annulus to 490 psi and tested for 15 minutes. At 15 minutes annulus pressure was 480 psi.

John stated the well has been shut down until they can complete work at site and it has been approved by the Division. He also stated they have not had 20 brine trucks per day arriving at site.

1357685049 2011-08-16 Salt Water Injection Well Violations Noted An inspection of the well revealed pump not being used for disposal at the time, unloading pad has several cracks in retaining wall and retaining wall for storage pit has several cracks. No pressure on annulas or injection side of well. Cracks need to be repaired and seal to prevent leakage from unloading pad and storage pit. A mini test need to be preformed on well.

The 420 psi to perform the test was never reached, the annulus pressures reached 200 psi and the pressures on the injection tubing reached 32 psi.
1367505416 2013-04-29 Salt Water Injection Well No Violations Attempted to test annulus, pumped 714.8 gallons fresh water into the annulus and was unable to gain more than 200 psi. Injection tubing pressure was monitored during the annulus testing and was observed to increase from vacuum to 43 psi. Testing was stopped and arrangements are being made for a service rig to come and work on the well. Injection valves are shut off. Accompanied by Inspectors Josh Garland and Jake Glascock.
1369935453 2013-05-30 Salt Water Injection Well No Violations I was on location with Jon Scott to witness the installation of the packer followed by a mechanical integrity test. On the first try, the packer would not set, so the operator had to trip out in order to retrieve the packer. They will not be performing the MIT today.

1372436018 2013-06-25 Salt Water Injection Well No Violations Witnessed installation of a new T607 Team Oil Tools Patriot Packer with a new Valve T574-20A DFV Ball valve installed above the packer. New 8 round J-55 2 3/8 upset tubing. Packer set at 1775.9 pulled 8,000 lbs over string weight. Stung out of valve and circulated fresh water into a lined pit. Annulus was then filled with 18.58 barrels fresh water and 5 gallons of inhibitor. Tubing was stung back on to the valve. Multiple attempts were made to test the annulus. All tests failed due to air and foam from the inhibitor. Attempts were made to release the air none could produce a positive test. John Schneider decided to postpone the test and try to release the air over a couple of days.

1399402338 2014-05-06 Salt Water Injection Well No Violations Witnessed a reconditioned T607 Team Oil Tools Patriot Packer set in well on 1782 feet of 2 3/8 upset tubing. Packer set at 2200 lbs and then the tubing was unseated from the packer. 18.06 Barrels of fresh water mixed with roughly 5 gallons of corrosion inhibitor was circulated through the annulus to the surface. Then the tubing was stung into the packer and a pressure test was administered. The annular space held 500 psi for the required 15 minutes with an acceptable decrease in pressure at the well head.
