0352904953 | 2018-07-02 | Production Wells | No Violations | Following up to a brine release on the Johnson lease I took two soil samples and mixed deionized water with them to field test for chlorides after soil was removed. I got results of 2.8, equivalent to 402 ppm and .9 which is lower is a reading lower than quantabs would read less than 286 ppm. |
0440720600 | 2018-08-24 | Production Wells | No Violations | None of the work discussed on the 14th of August has been completed. Remediating soil where brine is traveling alongside the 2 inch drain, using bentonite to stop the drainage and reseeding with winter wheat. I called PDQ Sanitation and left a message. |
0553873887 | 2018-10-12 | Production Wells | No Violations | Well is equipped with a pumping unit, tubing, rods and a engine converted to run on natural gas with an automatic start switch. |
0586282594 | 2017-08-23 | Production Wells | No Violations | I texted Mark Abduella and asked that when he had a chance, to call me. He called me approximately 30 mins later. We discussed the rest of the clean up and the NOVs that are enroute to him right now. He explained that he had a company making up stickers to place over the identifications that are up now. |
0606323874 | 2018-08-21 | Production Wells | No Violations | Nothing that was discussed a week ago has been completed. The removal of the remaining affected soil, the sowing the area with winter wheat, and the bentonite in the water meter housing that is housing the valve. I texted Mark Abdulla to remind him that it needs completed. |
0714476121 | 2017-06-21 | Production Wells | No Violations | Work has not begun at the site. However, the dike is going to be lined and the tank battery will be inside the dike instead of the downslope design. |
0902564550 | 2019-04-22 | Production Wells | No Violations | My inspection revealed the pollution that escaped the dike had been removed. The area has vegetative growth. This satisfies the compliance notice. |
0930578687 | 2017-10-03 | Production Wells | No Violations | Dike impoundment has been vacuumed out. Work to remediate below the dike should start soon. |
1007939456 | 2004-04-15 | Production Wells | No Violations | FOLLOW UP TO NOV. Well is in noncompliance. Oil pooling at the well head. Well has ID. Updated well latitude and longitude. Tank battery contains a frac tank and seven 100 bbl tanks, two of which are disconnected. Recorded the coordinates of the tank battery location. |
1036998297 | 2016-12-13 | Administrative Inspection | No Violations | I called Mark Abduela the owner of PDQ at approximately 2:23 pm and spoke with him about the Compliance notice that was issued and what needed done to meet compliance at this time. |
1116245636 | 2018-08-16 | Production Wells | No Violations | Nothing has been completed that was talked about Monday night. |
1157286049 | 2018-08-27 | Production Wells | No Violations | There has been no seed applied to the small area of the location where no grass has grown. There has been no affected soil removed. There was no bentonite placed in the water meter housing where the valve is located. There was no standing water to test for chlorides. |
1172405909 | 2006-05-04 | Production Wells | No Violations | FOLLOW UP TO NOV. The well is in compliance. |
1269i485 | 2001-11-5 0:00: | PW | No Violations | |
1472530308 | 2016-12-16 | Production Wells | No Violations | Checked on status of clean up. Nothing has begun. |
1480837556 | 2018-06-27 | Production Wells | No Violations | I arrived on location accompanied by Supervisor Eric Conrad. We went to take samples. We sampled about 90 feet uphill from the tank battery to set the baseline for the samples. Then I collected a sample at the end of the dike where it was cut open. I then sampled where the two brine scalds converge to form one scalded area approximately 60 feet from the first sample. I then dug a third sample from where the dike previously ran over. I then took pictures of the sample locations and bottles filled with the samples. Earlier myself and Eric met Mark Abuella on location and field tested the water that accumulated in the dike with Quantabs and got results of in excess of 2500 ppm. |
1501270989 | 2001-11-05 | Production Wells | Violations Noted | FOLLOW UP TO NOV # 1269i485 An inspection of the well revealed that there is No ID at the well and oily soil around and below well head. Need full ID at well and oily soil cleaned up. |
1503492854 | 2017-08-23 | Production Wells | No Violations | Received a text last night saying the leaking elbow had been fixed. I traveled to the site and found that indeed the elbow was removed from the plumbing of the tanks. However the dike is still suspect and there are brine scalds downslope of the dike. |
1528827498 | 2018-06-26 | Production Wells | Violations Noted | Inspection of tank battery found that the dike below the tank battery had been cut open by an excavator. There was oil around and on sixth tank in the tank battery, indicative of an overflow event. The dike is empty of fluids. The brine scald starts to the south of the dike then runs down slope to the west of the dike. The brine scald and water flow from the release traveled at least 490 and averages about 15 wide. At 490 with a conductivity probe we had results of 3.43 and 3.93 mS/cm. There were also oily substances found along the path of the scald. Baseline readings were .15 mS/cm. I spoke with Mark Abduella this morning 6/27/18 at 8:19 am and discussed the violation. We also discussed the required remediation necessary. I was accompanied to this location by Emergency Response Coordinator Nathan Lane. This incident was reported to the Duty Officer for the Emergency Response Group. |
1536075289 | 2018-09-04 | Production Wells | No Violations | Bentonite has been placed in the water meter box housing the valve. This should allow the area to dry up to be excavated outside of the drain line. |
1538406397 | 2018-10-01 | Production Wells | No Violations | The affected areas were mulched with straw. The level of water in the dike was significantly lower. |
1542387858 | 2018-11-16 | Production Wells | No Violations | The affected area has vegetation starting back. Violation is resolved. |
1670901499 | 2007-01-31 | Preliminary Restoration | No Violations | Preliminary restoration meets all requirements. |
1682292160 | 2018-08-14 | Production Wells | No Violations | I spoke with Mark Abdella last night who said the track hoe was still down, a problem with the tracks, and said he was going to have one of his helpers work on the well today. Mark also said he was going to try to put bentonite in the pit to seal up where the water is getting around the pipe. I also discussed reseeding the location. He said he would get some winter wheat and put down on both locations. |
1703277166 | 2018-08-14 | Production Wells | No Violations | No further work to remediate the location has been completed today. The person who was supposed to start on this today went by while I was on lunch. |
1748672444 | 2018-01-30 | Production Wells | No Violations | The area that was remediated still looks good. There have been several tanks replaced in the tank battery. The well was producing using a pumping unit, tubing, and rods. |
1847723883 | 2018-07-31 | Production Wells | No Violations | Inspection of the area remediated revealed brine at the end of the dike drain that maxed out high range quantabs at 6280 ppm. There was also a bag of lime at the end of the drain pipe. There was an obvious area that was moist about 3 feet wide by 10-12 feet long. I called Mark Abduella who said he would dig up the affected soil and he concluded that it must be old contamination chasing the drain pipe under and then out around the outside of the drain. This would appear to be what is happening at this location because the drain isnt through the liner in the dike yet and the end of the dike drain ends about 75 feet farther downslope. |
1852035692 | 2016-12-08 | Production Wells | Violations Noted | Upon inspection of the tank battery 2 active leaks were discovered. One from an elbow connecting 2 100 barrels, the other from a valve. The containment dike was field tested with quantabs and revealed that the dike contained water with more than 600ppm chlorides. The leaks shall be fixed and the brine water in the dike shall be removed. The tank battery also had a tank that appeared to have run over or the lead line leaked and oily substance was found around the tank. The contaminated soil will need removed and the site of the contamination will need seeded and mulched to allow vegetation to reestablish. I was accompanied by Nate Lane the emergency response team member for the area. I called the owner and reported my findings and he said he would be out later today or the next to rectify the situation. |
1945457385 | 2018-12-05 | Production Wells | No Violations | The dike was emptied out and presumably hauled out with a water truck and disposed of properly. The reclaimed area was still supporting vegetation. |
1987664191 | 2018-07-09 | Production Wells | No Violations | The area that was remediated still lacks and mulch or straw and is extremely dried out. This will not help the seed to start growing. The area will need mulched. I texted Mark Abduella and let him know this needed completed. |
