The 30-day report is not required unless chief requests a follow-up report. If a 30-day report is requested by chief for this non-compliance use this language also:

(D) Follow-up reporting: If the incident involves a release of a substance specified in paragraph (A)(4), (A)(6), (A)(7), or (A)(10) of this rule, the reporting person also shall submit to the division of oil and gas resources management a follow-up report no later than thirty days after the release. If necessary to document factors that contributed to an occurrence specified in paragraph (A) of this rule and its final resolution, the chief may request a follow-up report. 0352904953 2018-07-02 Production Wells No Violations Following up to a brine release on the Johnson lease I took two soil samples and mixed deionized water with them to field test for chlorides after soil was removed. I got results of 2.8, equivalent to 402 ppm and .9 which is lower is a reading lower than quantabs would read less than 286 ppm.

1480837556 2018-06-27 Production Wells No Violations I arrived on location accompanied by Supervisor Eric Conrad. We went to take samples. We sampled about 90 feet uphill from the tank battery to set the baseline for the samples. Then I collected a sample at the end of the dike where it was cut open. I then sampled where the two brine scalds converge to form one scalded area approximately 60 feet from the first sample. I then dug a third sample from where the dike previously ran over. I then took pictures of the sample locations and bottles filled with the samples. Earlier myself and Eric met Mark Abuella on location and field tested the water that accumulated in the dike with Quantabs and got results of in excess of 2500 ppm.

1503492854 2017-08-23 Production Wells No Violations Received a text last night saying the leaking elbow had been fixed. I traveled to the site and found that indeed the elbow was removed from the plumbing of the tanks. However the dike is still suspect and there are brine scalds downslope of the dike.
1528827498 2018-06-26 Production Wells Violations Noted Inspection of tank battery found that the dike below the tank battery had been cut open by an excavator. There was oil around and on sixth tank in the tank battery, indicative of an overflow event. The dike is empty of fluids. The brine scald starts to the south of the dike then runs down slope to the west of the dike. The brine scald and water flow from the release traveled at least 490 and averages about 15 wide. At 490 with a conductivity probe we had results of 3.43 and 3.93 mS/cm. There were also oily substances found along the path of the scald. Baseline readings were .15 mS/cm. I spoke with Mark Abduella this morning 6/27/18 at 8:19 am and discussed the violation. We also discussed the required remediation necessary. I was accompanied to this location by Emergency Response Coordinator Nathan Lane. This incident was reported to the Duty Officer for the Emergency Response Group.

1682292160 2018-08-14 Production Wells No Violations I spoke with Mark Abdella last night who said the track hoe was still down, a problem with the tracks, and said he was going to have one of his helpers work on the well today. Mark also said he was going to try to put bentonite in the pit to seal up where the water is getting around the pipe. I also discussed reseeding the location. He said he would get some winter wheat and put down on both locations.

1748672444 2018-01-30 Production Wells No Violations The area that was remediated still looks good. There have been several tanks replaced in the tank battery. The well was producing using a pumping unit, tubing, and rods.
1847723883 2018-07-31 Production Wells No Violations Inspection of the area remediated revealed brine at the end of the dike drain that maxed out high range quantabs at 6280 ppm. There was also a bag of lime at the end of the drain pipe. There was an obvious area that was moist about 3 feet wide by 10-12 feet long. I called Mark Abduella who said he would dig up the affected soil and he concluded that it must be old contamination chasing the drain pipe under and then out around the outside of the drain. This would appear to be what is happening at this location because the drain isnt through the liner in the dike yet and the end of the dike drain ends about 75 feet farther downslope.
1852035692 2016-12-08 Production Wells Violations Noted Upon inspection of the tank battery 2 active leaks were discovered. One from an elbow connecting 2 100 barrels, the other from a valve. The containment dike was field tested with quantabs and revealed that the dike contained water with more than 600ppm chlorides. The leaks shall be fixed and the brine water in the dike shall be removed. The tank battery also had a tank that appeared to have run over or the lead line leaked and oily substance was found around the tank. The contaminated soil will need removed and the site of the contamination will need seeded and mulched to allow vegetation to reestablish. I was accompanied by Nate Lane the emergency response team member for the area. I called the owner and reported my findings and he said he would be out later today or the next to rectify the situation.
