I observed an ECS&R Truck, License number AF-60752 Pull up to the K&H # 1 unloading pad and after interaction with the K&H staff The ECS&R truck pulled to the K&H Treatment facility unloading pad. I observed that the ECS&R truck was not labeled with a UIC number or Brine. The truck was labeled with Residual Waste, Waste Oil and a placard 1993. I asked the driver what he was hauling and to see the brine hauling log. I asked the driver why the truck was not labeled correctly. He said the truck is not used to haul brine and is used to clean out tanks. The 100 barrels of water on the truck originated from Chesapeake Energy Well # 47-069-00082. The driver told me that the water was from a tank cleaning job. I called ECS&R and Talked to Brandon and informed him that they must label all trucks hauling into either a UIC injection facility or a Treatment Facility. I gave the truck driver a copy of the requirements for brine haulers. We discussed the 4 lettering/numbering in reflective paint for the UIC number and Brine. ECS&R is a registered brine hauler UIC # 25-545. According to DOGRM Engineer Ron Trivisonno the Order to operate a Treatment Facility had not been issued to K&H Partners LLC. The fluid in the ECS&R Truck was unloaded at the treatment facility. The treatment facility has a centrifuge, 3 tanks, 1 half tank (located below the centrifuge) a clam shell roll off box, and another roll off box with fly ash used to mix with fluids prior to placing in the clam shell roll off box and hauling to the landfill. All oil based materials are separated then mixed (solidified) and sent to the landfill. Any water that is extracted from the process is transported by truck to the K&H # 1 surface facility for injection. |

An area was able to be delineated South East of the pump house and down slope of the chain link fence approximately 35 feet by 15 feet. K&H staff are currently remediating the area. The 3 polyflow line was pressurized to 750 psi to stretch the pipe which was installed through a 6 PVC conduit from the well to the pump house. I witnessed the 3 line being cut off and the 3 male adapter being installed. The fitting was crimped using a specialized hydraulic press. Both ends of the injection line have a crimped stainless steel adapter installed. Threaded stainless steel schedule 40 tubing was installed from the adapter to the wellhead. Wellhead has two 2 check valves, two 2 hammer unions, one 2 master valve, a 2tee with a 2 plug installed, and a 2 tee with a 2 bull plug tapped for 1/4 with a 1/4 needle valve and gauge. Dave Chandler from Sattler Pump Solutions was on site to test a centrifugal pump on the 3 injection line. The pump was installed with quick connection hoses. The pump had a shut off pressure of 200 psi. While attempting to pressurize the 3 injection line one of the quick connect hoses burst at 150 psi. the water was shut off and the test was postponed. |

I observed that the ECS&R truck was not labeled with a UIC number or Brine. The truck was labeled with Residual Waste, Waste Oil and a placard 1993. I asked the driver what he was hauling and to see the brine hauling log. I asked the driver why the truck was not labeled correctly. He said the truck is not used to haul brine and is used to clean out tanks. The 100 barrels of water on the truck originated from Chesapeake Energy Well # 47-069-00082. The driver told me that the water was from a tank cleaning job. I called ECS&R and Talked to Brandon and informed him that they must label all trucks hauling into either a UIC injection facility or a Treatment Facility. I gave the truck driver a copy of the requirements for brine haulers. We discussed the 4 lettering/numbering in reflective paint for the UIC number and Brine. ECS&R is a registered brine hauler UIC # 25-545. According to DOGRM Engineer Ron Trivisonno the Order to operate a Treatment Facility had not been issued to K&H Partners LLC. The fluid in the ECS&R Truck was unloaded at the treatment facility. The treatment facility has a centrifuge, 3 tanks, 1 half tank (located below the centrifuge) a clam shell roll off box, and another roll off box with fly ash used to mix with fluids prior to placing in the clam shell roll off box and hauling to the landfill. All oil based materials are separated then mixed (solidified) and sent to the landfill. Any water that is extracted from the process is transported by truck to the K&H # 1 surface facility for injection. |

Fluids were being transferred from the filter press unit to a vacuum truck on the pad. The transfer was not being monitored continuously and the fluid level in the truck filled up to the point of overflow and caused the pressure relief valve to open. The pressure relief valve sprayed fluid (oil and brine) over the side of the truck onto the pad and gravel access road. Less than 1 barrel of fluid was estimated to have been released. The weather conditions were rain and wind. The access road was flowing rain water runoff. The oil and brine mixed with this flow and extended approximately 118 by 13. The contamination was contained with clay soil, booms, pads, and absorbent material. A dry Vac truck was utilized to remove the oil and all fluids at the surface. All solids were collected and placed in a roll off box. Contamination did not extend past the 118 by 13 area. Contamination did not reach the intermittent stream down slope of the incident. |
