0389075327 2014-07-23 Casing String Witnessed No Violations I was on location to witness the cementing of the 20 conductor casing. The joints are butt welded and the casing is set open-ended on the bottom of a 26 hole that is 120 in depth. Holes have been cut in the casing 5-6 off bottom to allow fluid circulation. O-Tex Pumping cemented the casing with 47 barrels of 15.6lb/gal, Class H slurry. 20 barrels of cement slurry circulated to the surface. See cement report for more details on this job. No violations.
0486955474 2014-10-11 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing. Nomac Rig #73 drilled a 12.25 hole to a depth of 2411 and Wyoming Casing ran 2400 of intermediate casing into the hole. O-Tex Pumping rigged up and cemented the casing with 185 barrels of Class H cement. 24 barrels of cement circulated to surface and weighed 15.3lbs/gal on the pressurized scales. No violations. 0537053860 2015-10-08 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0554198658 2014-07-22 Drill / Deepen / Reopen No Violations I was on location to inspect the drilling status of the 26 conductor hole. Top Hole Drilling was auguring 3-120 holes for this well and the #s 3H & 4H. I spoke with Gene Belsom (Chesapeake) and he said they would be ready to cement tomorrow afternoon. Four other conductor holes have been drilled and cemented on this pad (#s 1H, 2H, 6H, and 10H). No violations.
1332550884 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was in good repair, but was unlocked at the time of inspection. The #8H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and equipped with locks on the man door. The man door was open at the time of inspection, but Chesapeake personnel were on site. The equipment on the pad consists of the following:

9 -400 bbl production tanks, three labelled as containing produced water, six labelled as containing condensate. 1- Low pressure vertical separator located inside of the dike of the tank battery. 7 Producing wells 7 Sand separators 7 Gas Processing Units 5 Line Heaters 1 Heat Treater 1 Vapor Recovery Unit 2 Bullet Tanks

1422993968 2014-10-09 Drill / Deepen / Reopen No Violations I was on location to inspect drilling operations of Nomac Rig #73. The rig spudded the 17.5 surface hole at 11:00am. I inspected the location and it is in good condition. All drilling equipment is on containment. No violations.
1423163393 2014-10-09 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac Rig #73 drilled a 17.5 hole to a depth of 861 and Wyoming Casing ran 850 of surface casing into the hole. O-Tex Pumping rigged up and cemented the casing with 164 barrels of Class H cement. 37 barrels of cement circulated to surface and weighed 15.5lbs/gal on the pressurized scales. No violations. 1890290289 2014-10-09 Drill / Deepen / Reopen No Violations I was on location and received a noise complaint (#4830) from the Uniontown Field Office. The complaint was filed by Mr. Terry Howell about this well pad stating that he and his wife have not been getting good sleep for over a month due to the drilling operations. I contacted Mr. Howell at 12:45pm and met him at his home at 12:50pm. From his front porch, the home and the drilling pad appear to be at the same elevation and is approximately 700-800 from the southwest corner of the drilling pad. Mr. Howell said he has spoken with the company men on location and said they have been good about keeping the lights from shining into his yard. He said the noise is affecting his wifes sleep. The rig moved in the first week of August and began drilling on 8/8/14. I told Mr. Howell that they are currently drilling the seventh of eight wells on this pad and they should be finished in 2-3 weeks. At this point, there is not much I can do about the rig noise, but I will ask them to control the light towers and refrain from using the rig horn unless absolutely necessary. I will talk to Chesapeake about installing several sound panels on the southwest corner of the pad prior to conducting completion operations. This complaint will continue until the rig is moved.
