0297902874 2015-02-13 Casing String Witnessed No Violations I was on location to witness the cementing of the 5.5 production casing. Nomac drilled the 8.5 curve and lateral to a total measured depth of 15,922 and Wyoming casing landed the 5.5 production casing at 15,915. Nomac 71 circulated an entire hole volume and Schlumberger rigged up their iron to the Black Hawk tool and pressure tested the lines. They then mixed and pumped 60 barrels of 12.5lb/gal, Mudpush Express and followed that with 312 barrels of lead cement and 443 barrels of tail cement. They released the plug and displaced the plug to 15,823 with 350 barrels of water. The plug landed at 2925psi and the pressure was increased and held at 3400psi for five minutes. The pressure was released and the floats held with 5.5 barrels flowing back to the truck. 10 barrels of lead cement circulated to surface. The casing was rotated throughout the job. 0517237716 2015-04-15 Preliminary Restoration No Violations I was on site to check the status of this 5 well pad that was drilled from 1/7/15 to 3/22/15. The permit #s for the other horizontal wells on location are 2629, 2652, 2653, & 2654. The wells have not been stimulated and there are no production facilities or related equipment on location. All drilling equipment has been removed and the entire pad is fenced and all access gates are locked. The cellars are full of water and covered by steel grates. All the annuli are equipped with pressure gauges but are underwater and not readable at this time. The pad berms need to be stabilized with vegetation to prevent excessive erosion on the inside of the pad. Compost filter socks are still in place around the exterior of the pad and are functioning properly. During my inspection of the wellheads, I noticed visible bubbling emanating from the flange of a gate valve. I notified the Chesapeake Field Representative, Brett Farnsworth, of the leak and he is going to have someone look into it. Preliminary restoration will be reviewed after completion of the wells and the construction of the surface facilities. No violations.
0947628206 2015-02-10 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac #71. They are currently drilling the 8.5 lateral at a total measured depth of 9,745. The location is in good condition and all drilling equipment is on containment. No violations.
1317831100 2015-01-12 Casing String Witnessed No Violations I was on location to witness the setting of the conductor casing. Top hole Drilling augured a 26 hole to a depth of 120 and sat 120 of 20 casing on bottom with holes cut 5 off bottom to allow circulation. Express Energy Services cemented the casing with 39 barrels of Class A cement slurry. 9 barrels of cement circulated to the cellar. See casing ticket for more details on this job. No violations.
1548756472 2015-02-04 Casing String Witnessed No Violations I was on location at 4:00am to witness the cementing of the 13.375 surface casing. Nomac Rig #71 spudded the 17.5 surface hole on 2/03/15 and drilled to a depth of 668. Wyoming Casing rigged up and ran 13.375 surface casing to a depth of 658. Schlumberger rigged up the cement head, tested the lines, and pumped 25 barrels of gel and 10 barrels of water ahead of the slurry. They then mixed and pumped 131 barrels of cement slurry, dropped the plug, and displaced the plug to the collar with 94 barrels of water. The plug landed at 350psi and the pressure was increased and held for 5 minutes at 880psi. The pressure was released and the floats held with 0.5 barrels flowing back to the truck. 34 barrels of cement circulated to surface. See cement ticket for more details on this job. No violations.
1927923912 2015-11-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.
