-Tex established circulation and conditioned the wellbore with 30 bbls of gel spacer followed by five barrels of water. I witnessed good circulation during the job. As reported on cement job log, O-Tex pumped 96 bbls of cement slurry. The cement slurry was mudscaled at 15.6 pounds per gallon (ppg). The cement slurry was displaced with 65 bbls of water. 19 barrels of cement returned to surface and I witnessed returns at the shakers. The plug was landed at 240 psi and increased to 750 psi and held pressure for five minutes. All floats held, and 0.5 bbls bled back to surface.

-Tex was the cementing contractor. I witnessed good circulation during cementing. O-Tex pumped 18 bbls of water to establish circulation, followed by 49.2 bbls of Class A cement. I witnessed the cement being mudscaled at 15.6 ppg. The slurry was displaced with 26 bbls of water. Eight bbls of returns circulated to surface. The company man expects to wait at least twelve hours before resuming drilling operations. I verified through the O-Tex cement job report that a compressive strength of 500 psi is reached after nine hours.

1193824017 2017-01-11 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The pad houses three producing well. This well is open flow with a wellhead Christmas tree assembly. The surface--lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

Nine Storage tanks, six labelled as containing condensate, and three labelled as containing produced water1 Producing well one low-pressure vertical separator three sand separators three gas processing Units one combuster unit one oil stabilization unit Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

--55 casing were ran as reported on the cement tally. The job commenced and was pumped as plan with 157 bbls of class A cement. The cement sample was weighed at 15.8 ppg before being pumped downhole. I witnessed good circulation at various times throughout the cement job. Approximately 33 bbls of cement circulated to surface. I witnessed cement returns at the shakers. I verified through O-

1637359955 2017-11-01 Drill / Deepen / Reopen No Violations I was on site to check the status of completion operations. I spoke to the consultant on site, who informed me that hydraulic fracturing was completed on the three drilled wells on the pad (#6H, 7H, & #9H) wells during the week of 10/23/2017, and Pro-Frac was the contractor. A service rig was in the process of drilling out plugs on the three wells, and flow-back operations were expected to take place within the next couple of weeks.
2075682626 2017-09-14 Production Wells No Violations I was on site to check the status after Patterson #571 moved off site. The rig and all drilling equipment has been removed from site. I have not received indication from the operator as to when completion operations will take place. No violations were noted at the time of inspection. I performed a walk-around inspection of the pad, and did not locate any issues of sedimentation and erosion. No violations were noted at the time of inspection.
