0236458774 | 2017-09-20 | Production Wells | Violations Noted | The purpose of my inspection was to conduct a status check on well #2-0573. The lease road to the well remained in good repair. A review of RBDMS indicated the Owner was current with reporting production. The well and production equipment were situated inside a chain link fence. The base of the location was lined with crushed gravel. No identification was present at the well head. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. The base of the plunger catch tube was leaking production fluid and gas. Live production fluid was present on the well head assembly and staining the ground. The fluid was not mobile. The condition of the well head indicated the leak was not recent. I also observed the hammer union on the lower U-tube assembly to be leaking production fluid onto the ground. Production equipment inside the fence consisted of one (1) vertical separator and one (1) 100 bbl production tank. No identification was present on the separator. No earthen dike was present around the separator or production tank. I observed residual leaking from a hammer union on the flow line leading to the separator that was staining the ground. I observed residual leaking from the separator dump valve and pressure supply drip supply lines. The leaking was not live or mobile. No fluid was collecting on the ground. No identification was present on the production tank. The tank body remained in good repair. I observed evidence of overfilling of the tank that was staining the side of the tank and on the ground adjacent to the tank. Valves were closed and plugged. Operation of well #2-0573 was causing pollution and contamination in violation of Ohio Revised Code 1509.22(A) and Ohio Administrative Code 1501:9-1-07. The well head assembly was insufficiently equipped to prevent the escape of oil and gas in violation of Ohio Revised Code 1509.20 and Ohio Administrative Code 1501:9-9-05(A)(1). The Operator failed |
0532121144 | 2017-10-16 | Production Wells | No Violations | The purpose of my inspection was to follow up on compliance notice #236458774. I received a phone call from Mr. Don Bandy, who is the operator of well #2-0573. Mr. Bandy stated that he had repaired the well head leak and cleaned the adjacent area up. He continued that the flow line unions and valves were tightened to prevent further leaking. Correct identification was posted on the production tank stairs. Mr. Bandy requested that the earthen dike requirement be reconsidered due to the lack of a clear and present hazard and the financial status of the well owner, Mile Branch Grange. The well and production equipment was located over 2800 feet from the nearest surface water source as measured on the ODNR-DOGRM oil and gas well locator. Topography was flat. The location was also located away from the nearest public roadway. The location lacks a clear and present hazard therefore the earthen dike requirement was waived. At the time of my follow-up inspection the well head leaks had been repaired and free product cleaned from the well head. Contaminated material around the well head was removed and new gravel applied. No further action was necessary. |
