0056626984 2004-05-04 Production Wells Violations Noted The tank battery is shared with permit 0898 and consists of 1-210 bbl storage tank, 1-buried concrete salt water tank and a dike.
1247038979 2017-07-03 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0899. The lease road, well and tank battery were stable. The 4.5 production casing was equipped with an electric powered pump jack. Correct and legible identification was present. The stuffing box had leaked production fluid over the well head and onto the ground. The fluid was contained to the well head area and was not migrating. A casing gauge indicated 95 psi and a tubing gauge indicated 105 psi. The tank battery was shared with #2-898 and consisted of one (1) horizontal separator and one (1) 210 bbl production tank. The separator was in good repair and contained a current 31 day production chart in the meter house. An earthen dike that was stable and holding freshwater was constructed around the production tank. Correct and legible identification was present on the tank body. Valves were closed and plugged. No leaks were observed. While on location I called and spoke with Dorfman Production Manager Paul Shaw to make him aware of the well head leak. Mr. Shaw stated he would schedule the lease operator to inspect the well head and make necessary repairs to stop the leaking. No violations were observed during this inspection.
1643506363 2015-06-30 Production Wells No Violations I was on site to conduct a status check. Identification was posted at the tank battery. The tank battery is shared between Stoffer #6 (20890) and the Stoffer #7 (20899). The tank battery consists of a 210 bbl production tank and a horizontal separator. There is a meterbox with a current 7 day chart showing production. A dike is present and in good repair. The wellhead is equipped with an electric pumpjack. And a domestic supply line. Access road and site are stable. No violations were observed.

1875913125 2017-07-24 Production Wells No Violations The purpose of my inspection was to document repairs to the leaking stuffing box observed during inspection #1247038979 on 7.3.2017. I had previously spoke with Dorfman superintendent Paul Shaw who stated the lease operator had adjusted the stuffing box. At the time of my inspection the stuffing box was still leaking gas. I called Mr. Shaw to communicate my findings. Mr. Shaw stated he would personally inspect the well head and determine what further action was necessary. I will schedule to re-inspect the well to document that the leak was repaired.
