| 1539783816 | 2018-10-16 | Exempt Well Pre Transfer | Violations Noted | The purpose of my inspection was to conduct an exempt domestic pre transfer inspection on well #2-0908. Access was through the driveway of 10592 S.R. 170 on the East side. No direct access existed to the well. An unlocked four sided chain-link enclosure was present around the well. No identification was present. No public or private structures were present within 200 feet of the well. The 7 surface casing head was above grade. The annular ports contained plugs. The 4.5 production casing was swaged down to a 1.5 flow line that contained a pressure regulator. The casing pressure gauge on the well head indicated 380 psi. Production valves were in the open position. No leaks were witnessed. No tanks or any other production equipment was present. Well #34-029-2-0908 was not in compliance with ORC 23 and OAC 1501:9-9-05(A)10 failure to legibly identify well. The owner shall legibly identify the well in a conspicuous place with the owner, well name and number, API well number and an emergency telephone number within seven (7) days of receipt of this compliance notice. The owner shall notify Inspector Kenneth Frye at 330-324-4545 that identification has been posted to facilitate a follow-up inspection. |
