0430951308 2017-12-12 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1255. A review of RBDMS indicated the owner was current with reported production. Access to the well and tank battery was stable and drivable. Correct identification was present. The 8.6265 well head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. No leaks were observed. A stable earthen dike that was free of fluid was present around the production equipment. Equipment within the dike consisted of one (1) vertical separator, one (1) 210 bbl production tank and one (1) concrete brine vault . The separator remained in good repair and contained a current 31 day production chart. The production tank remained in good repair and contained correct identification. Valves were closed and plugged. The vault was covered with snow at the time of inspection. The vault was holding a de minimis volume of fluid. No violations were observed during this inspection.
0545781594 2014-10-01 Production Wells No Violations I was on site to conduct a status check. Identification was posted on the tank battery and wellhead. The tank battery consists of one 100 bbl production tank, a brine vault, and one vertical separator. The tank valve is plugged. There is a meter box with a current chart showing production. The dike is present and in fair condition. The wellhead is equipped with an electric pumpjack. The access road and site are stable.
0548312712 2012-01-11 Production Wells No Violations The dike was restored to a clean and functional condition. All oil and paraffin in and around dike has been cleaned up. All contaminated soil within an approximate 50 radius of tank battery has been removed for disposal at an approved landfill. The residual oil/water has been vacuumed from field tile ditch. A siphon dam was installed in this ditch and lined with a new boom and absorbent pads to filter out all remaining risidual oil. Enervest will monitor the water levels in this ditch, will continue to filter drainage through siphon dam/booms/pads and vacuum water in sump as needed until all evidence of impact is removed. They will notify me to witness the installation of new field tile and the closing of the ditch.

0944632835 2012-01-10 Production Wells No Violations The dike area was cleaned up, a new gravel base was installed, the separator had been cleaned of all oil and paraffin and a new stock tank was in place and fitted with a spark arrestor. There are still areas surrounding tank battery that require attention. I left instructions to remove more topsoil in an approximated 50 radius around dike. I also instructed the clean up crew to have a pump truck come out immediately to vacuum oil and water from field tile ditch. Will follow up on 01/11/2012.
1326115783 2012-01-06 Production Wells No Violations I was called out to cover clean up efforts after the stock tank assocoated with this well exploded. This explosion was attributed to static electricity. There were no electrical lines nearby and no fire was associated with the explosion. Tom Basick, of Enervest Operating, was on location and reported approximately 100 barrels of crude oil had escaped in the explosion. Clean up was already in process upon my arrival. Tom Basick estimated that 90% of the oil released in the explosion was contained within the earthen tank battery dike. This crude oil collected in the eastern end of the dike. There was crude oil and paraffin coating the dike walls and in localized pools approximately 0 - 10 meters around dike. Approximately 50 of field tile was excavated to prevent oil migration. The excavated field tile ditch was lined with absorbent pads and booms. A sump was also excavated at the far end of the ditch. The stock tank, which launched off of the tank pattery pad, landed in an open field approximately 168 from the tank battery. There was no property damage or personal injury related to this incident. There were no streams, wetlands, or waterways affected by this incident. At the time of my initial inspection, Tom Basick reported that approximately 90 barrels of crude oil were recovered by water truck vacuuming. Approximately 20 cubic yards of impacted soil was excavated for transport to an approved landfill. Enervest will continue clean up efforts over the weekend and I will follow up on Monday, Jan 9, 2012.
1326380874 2012-01-09 Production Wells No Violations Clean up is still in progress. A steamer unit was on location to clean the stock tank for transport and to clean the separator and all associated equipment. More oil and paraffin was found, surrounding the tank battery, after the snow cover melted. I walked the site with a clean up crew member to point out all areas that still need to be addressed. I recommended that several Rhus spp. Trees, adjacent to the dike, be removed in order to completely clean up all paraffin and crude oil outside of dike. I will follow up 01/10/2012.
1328279228 2012-01-31 Production Wells No Violations I received call from Tom Basick, of Enervest Operating (330-575-9248), to request that I inspect well site clean up before they move forward on reinstallation of field tile and closing of drainage ditch. On the day of my inspection, I found that the drainage ditch was free of oilbrine impact, as was the surrounding soil and dike area. I notified Tom Basick that the clean up of the drainage ditch and site passed inspection, and they (Enervest Operating) were authorized to reinstall the field tile, close the ditch and finish clean up. I will receive notification upon completion so I can conduct a final inspection of the spill clean up.
1714806541 2012-02-02 Production Wells No Violations I received notification from Tom Basick of Enervest Operating that the clean up of the tank explosion was complete on this well site. I conducted a final inspection of the site and found that the well is equipped with a pumping unit and the tank battery consists of one 100 barrel stock tank, an underground concrete brine storage tank and a separator. The dike walls were free of oil impact and the dike contained a new gravel base. All of the impacted soil surrounding the tank battery had been removed from the site, new field tile was installed and the drainage ditch closed. Proper identification was posted. No violations observed and this site clean up passes inspection.
