0709445348 2017-10-13 Plug / Plug Back No Violations The purpose of my inspection was to witness the location of the surface plug set on 10/12/2017. Access to the well remained stable and drivable. Chipco remained on location and was rigging down. I met Enervest The lined earthen pit was emptied and was being prepared for backfill. I met with Enervest Ohio North Superintendent Doug Beighle and Engineer Josh Schrock while on location. I witnessed the cement plug had dropped down approximately 48 from surface. No gas was observed. I will schedule to re-inspect the location for final restoration.
1207946430 2017-10-12 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging activities on well #2-1656. The top of the first plug was measured at 5700 below surface. The plugging crew moved up the well bore and telegraphed the rip tool at 3040 below surface. Attempts to part the casing were unsuccessful. Company representative Tim McConaha contacted Appalachian Well Surveys (AWS) to shoot the pipe off. I witnessed AWS run a bond and free point log. The free point in the well bore was determined to be 2800 feet. The casing was shot at 2800 feet and two joints were removed. Circulation was established prior to pumping the rip plug. 90 sacks of Class A cement scaled at 14.6 ppg were pumped and displaced with 39 barrels of freshwater. After setting the rip plug, the 4.5 production casing was removed to 1072. After establishing circulation, 60 sacks of Class A cement scaled at 14.6 ppg were pumped and displaced with 14 barrels of freshwater. After setting the surface casing seat plug, the 4.5 production casing was removed to 150. After establishing circulation 50 sacks of Class A cement scaled at 14.6 ppg were pumped and circulated to surface. After removing the remaining 150 of 4.5 production casing, the well was topped off with cement to surface. The plug was holding at surface. I will schedule to witness the plug location on 10/13/2017.
1330528562 2012-02-28 Preliminary Restoration No Violations On 2/28/2012, I performed a status check on the above well site. The well is in production, the site is reclaimed and identification signs are posted. The Preliminary Restoration is PASSED.
1470705291 2017-10-11 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging activities on well #2-1656. Enervest Southern Ohio Operations Superintendent Tim McConaha was on location. Mr. McConaha reported the tubing was installed into the well due to depth of 6143 feet and circulated with freshwater and 420 gallons of Sulfa-Clear to treat the observed hydrogen sulfide. The well was dead. 30 sacks of Class A cement mixed at 14.6 ppg was pumped through the tubing and displaced with 22.2 barrels of freshwater. I scaled a representative sample and found the cement to weigh 14.5 ppg. After removing the tubing operations would shut down until 10/12/2017. I will schedule to witness the remainder of plugging operations.
1482591344 2017-03-06 Production Wells No Violations The purpose of my inspection was to conduct a status check on well # 2-1656. A review of RBDMS indicates the Owner last reported production in 2014. Lease access was through a residential driveway that lead to a livestock pasture. The road was stable and drivable. The well was located in a livestock pasture. The area around the well was stabilized with well-established vegetation. A perimeter fence comprised of well tubing was constructed around the well. The 8.625 well head was partially exposed above grade. An exposed well head annular side port contained an open valve plumbed into the flow line. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the closed position. The polishing rod did not show signs of recent operation. Identification on the pump jack identified the former well owner. All other information was correct and legible. No audible or visual leaks were detected. An earthen dike was present around the production equipment. The dike was stable but worn due to the surface owner keeping a feed location in close proximity to the tank battery. A chain link fence was constructed around the production equipment to prevent access. The fencing remained in good repair. Production equipment inside of the fence consisted of one (1) vertical separator, one (1) 210 bbl production tank and one (1) 140 bbl brine tank. The vertical separator remained in good repair. The production tank contained correct and legible identification. The load line valve was closed and plugged. The drain line valve was closed and plumbed to the brine tank. The hatch lid was closed. I did not observe any leaks in the tank body. The brine tank remained in good repair. The drain line valve was closed and plugged. I did not observe any tank body leaks. At the conclusion of my inspection, I called and spoke with Enervest lease operator Steve Pegg. I questioned Mr. Pegg about the condition of the well and any plans to opera
1507742660 2017-10-10 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging activates on well #2-1656. The location was in a pasture field and remained stable. An earthen pit was dug and lined. I did not witness any shallow groundwater in the pit. Chipco was the plugging contractor. Enervest Southern Ohio Operations Superintendent Tim McConaha was on location. Surface equipment was removed and Chipco was removing rods and tubing when I arrived on location. Per Mr. McConaha reported hydrogen sulfide gas that was measured at 80 ppm prior to beginning activates. The crew was unable to pull the down hole pump free and proceeded to strip the rods and tubing from the well. The last four joints of tubing were packed with frac sand and rods had to be cut. Mr. McConaha reported having to receive replacement tubing and that the Clinton formation isolation plug would be set on 10/11/2017. I will schedule to witness the bottom plug on 10/11/2017. No violations were observed during this inspection.
1765207073 2018-10-04 Final Restoration No Violations The purpose of my inspection was to document the status of final restoration on well #2-1656. At the time of my inspection the former well location was graded to local contour. Vegetation was well established. The working pit was backfilled. The production equipment was removed and the former well location was graded to local contour. Vegetation was well established. The flow line was left in place. Per Enervest field superintendent Tim McConaha, the line was cleared prior to burial. No debris remained. Mr. McConaha needs to submit Form 5 to finalize reclamation. Final restoration was not passed at this time.
