0126846315 2006-02-14 Production Wells No Violations Inspected this date to determine the GPS reading and verify which permit number is correct for this well. The tank battery is shared with permit 61485 and consists of 1-70 bbl storage tank, 1-buried concrtete salt water tank and dike. The well is idle, no bond, no identification at the wellhead and tank battery.
0189862435 2017-05-09 Production Wells Violations Noted The purpose of my inspection was to conduct a status check on well #6--DOGRM. The well was equipped with a engine powered pump jack. The engine was not present. No audible or visual leaks were identified. The tank battery consisted of one (1) 60 bbl production tank and one (1) concrete brine vault. The side of the tank body remained in good repair. I did not observe any holes or leaks. The load out valve cross was closed and plugged. The drain valve was closed and plumbed to the brine vault. The brine vault was in good repair above grade. No evidence of overfilling was observed. The vault contained freshwater at freeboard. I observed water dripping from the load out valve. I collected a sample of the water and field tested the sample for pH and Chloride. The pH test strip indicated approximately 6.5. The low range chloride Quantab indicated 0. I called and spoke with Shawn Watson regarding the leaking valve and vault. Mr. Watson stated he would work on repairing the valve that day and would get the vault scheduled to be emptied. Correct and legible identification shall be posted at the well head and tank battery within 14 days in accordance with 1501:9-9-05 (A)(10). The well was incapable of production. Under 1509.62 (A)(1) the owner of a well that has no reported production for two consecutive reporting periods as reported in accordance with section 1509.11 of the Revised Code shall plug the well in accordance with section 1509.12. 0785641507 2015-03-31 Production Wells No Violations I was onsite to conduct a status check with Inspector Kevin Perry. The wellhead is equipped with a gas powered pumpjack. The well consists of 6.25 casing, 2.375 piping and rods. No identification was posted. Well not capable of production.
0955321580 2019-01-17 Production Wells No Violations The purpose of my inspection was to follow-up on compliance notice #189862435. RBDMS indicated the Owner reported zero production in 2017. The location remained stable. Correct identification was not present. No audible or visual leaks were observed. The tank battery consisted of one (1) stock tank and one (1) concrete brine vault. The tank remained in good repair. I did not observe any holes or leaks. Load out valves were closed and plugged. The hatch lid was closed. The brine vault contained water that was frozen on the surface. I spoke with Shawn Watson on 01/25/2019 regarding the identification. Mr. Watson reported posting identification at the well and would revisit the location to replace it. He continued that he was working to finance plug and abandonment on the well.
1740861704 2014-07-02 Production Wells No Violations I was on location to investigate an idle well with no registered owner. I found during my inspection a motor powered pumpjack. The well was not in production. There was a 6.25 casing, 2.375 piping and rods. There was 2.375 flow line extending from the well. I noticed no identification sign. There was no lease road. Ownership of the well was under review.
