0899645804 2017-02-13 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #6-2541. A review of RBDMS indicates the Owner is not current with reporting production. A review of Columbus RBDMS indicates the Owner is deceased and the well is currently held by the Estate of the Owner. The location of the well and tank battery are in woodlands adjacent to Little Yellow Creek, a direct tributary to the Ohio River. At the time of my inspection no identification was present at the well or production tank. At surface, 8.75 conductor casing is driven in place. The surface casing is equipped with a plunger lift style tubing well head. No Demco gate valve is present. An annular gauge on the casing indicated 0 psi. A pressure gauge on the tubing head indicated 300 psi. A 2.25 flow line is plumbed out of the tubing head to riveted steel production tank. I observed a hole in the side of the flow line that was flowing gas and had leaked production fluid to the ground surface. The area of contamination from the leaking flow line was approximately three (3) feet by six (6) feet. The flow line was plumbed to the top of a 40 bbl steel production tank. The production tank was positioned inside of an earthen pit. The hatch lid was partially open and the fluid level in the tank was near overflow. Production fluid was covering the sides of the tank and had contaminated leaf litter on the ground adjacent to the tank. Some rust scaling was observed near the bottom of the tank. No other active leaks were observed on the body of the tank. No fluid was pooling or migrating within the earthen pit area. A load line valve was closed and contained a plug. I will contact the Owners Estate to address posting proper identification, repairing the flowline leak and emptying the production fluid tank.
1054301463 2003-05-19 Production Wells Violations Noted It has been determined that Mr. Dale M. Chenault of 20488 Hammondsville Road, Wellsville, Ohio 43968 is the owner of this well. The lease name is George King. The well number is 1 (one). The well is idle with possible bad casing. Plug or produce the idle well. If the well is produced, post legible identification, post bond and show proof of insurance. If you chose to plug the well file for a temporary owner number that will be issued to plug only. Also file for a plugging permit. There is also oil contaminated leaves and dirt approximately 25 across that needs to be removed and properly disposed of. Attached you will also find an application for a permit to plug. Should you have any questions please dont hesitate to call me.

1233788985 2003-05-19 Orphan No Violations Received a call from Marlene Hall regarding a complaint from Sgt. Kenneth Biacco Jr. of the Columbiana County Sheriffs Office (330-424-9519). The land was sold at a sheriffs sale to Judd Norse (330-223-3406). Mr. Norse called Sgt Biacco to report that oil was gushing from the ground, getting into Little Yellow Creek which empties into the Ohio River. Called Sgt. Biacco for additional information and set up a meeting. Called Reggie Brown at OEPA Twinsburg to file report. He will have someone respond. Kurt Kollar-OEPA called and will meet on the investigation. Met with Sgt. Biacco and obtained a map and additional information. Arrived on site and found the location. Mr. Kollar arrived and a joint investigation was conducted. Found an old tank battery and well. Oiled leaf litter is present around the wellhead. The approximate size is 15 X 25. There is no active leak and no oil entering Little Yellow Creek. Oil & Gas records indicate that the lease was signed by George King to Charles Peters & Company in 1930. The oil well was drilled in 1931. Further investigation will be made to determine if there is a viable owner to go after to correct the problem or referral to the I&O Plugging Program.

The production tank was positioned inside of an earthen pit. The hatch lid was partially open and the fluid level in the tank was near overflow. Production fluid was covering the sides of the tank and had contaminated leaf litter on the ground adjacent to the tank. Some rust scaling was observed near the bottom of the tank. No other active leaks were observed on the body of the tank. No fluid was pooling or migrating within the earthen pit area. A load line valve was closed and contained a plug. Per conversations with ODNR-DOGRM Northeast Region Manager Thomas Hill and Enforcement Coordinator Rick Anderson, I will complete an idle well report for this well.
1927858460 2017-02-16 Production Wells No Violations I spoke with the Owners Estate Attorney David Larue concerning compliance issues observed during my field inspection. I communicated to Mr. Larue that the well required proper identification to be posted, the hole in the flow line needed to be repaired and the production fluid tank needed to be emptied. We discussed the current state of ownership and the desire of the Estate to absolve ownership of the well to settle the Estate. I communicated to Mr. Larue that the compliance issues would need to be resolved prior to moving forward with an ownership transfer or applying to plug and abandon the well. I suggested the Estate contact a well service company to assist in addressing current compliance issues and to evaluate the well for viability. I will follow up with Mr. Larue in a week to determine the progress in making the necessary repairs.
