0841117118 2014-06-17 Production Wells No Violations This is a producing pump jack well. There were no problems noted at the time of the inspection.
0937363396 2017-07-17 Production Wells No Violations On the day of my inspection I met Frank Riggle at the site to go over their progress on the clean-up. The line had been temporarily repaired with a compression fitting. The absorbent booms were still in place at the site of the leak, downstream and at the bridge on TR 21. There was very little standing oil left. The only oil that I found was collected against the boom at the bridge. There were clean absorbent pads down through the intermittent stream. Frank and I did see some oil sheen at the site where the line was cut. We determined it to be coming from oil residue in the grass and weeds at the mouth of the culvert. Frank claimed that he has been working with the Township trustees and they have agreed to dig a ditch to relocate the gathering line road crossing away from the culvert. He said that they will excavate any remaining oily soil and grass when they move the line. Frank agreed to continue to monitor the site and change pads as needed. They will leave the booms in place until the end of the week.

1500645337 2017-07-21 Production Wells No Violations On the day of my inspection I found that the boom at the bridge on TR 21 was still in place. There were some oil stains on the lower portion of the boom. There was also a large amount of foamy scum that had collected on the water surface behind the boom. I talked to Frank Riggle and he said that when he lays absorbent pads in the scum it disappears and the pads come back clean. I did not see any sheen in the water behind the boom. I suggested that they get in there and stir the foamy residue up with a shovel and then lay pads in it to determine if crude oil is collecting in the foam. The two booms and randomly places pads were still in place in the intermittent stream. There was a minimal amount of brown film collected against the booms. There was still a film on the water surface at the culvert where the line was cut. I didnt see a sheen but the water flow was slow and calm. Frank said that they rented a mini hoe for the weekend and they were going to remove the soil and vegetation around the perimeter of the leak site. He said that he would like to keep the booms in place over the weekend. I will return on Monday to follow up.
1501085705 2017-07-26 Production Wells No Violations I received a call from Frank Riggle to report that they had completed the clean-up over the weekend and they were removing the booms. On the day of my inspection I found that all of the absorbent pads and booms had been removed. The oil impacted soil and vegetation around the origin of the leak had been excavated. I found no sheen or oil residue in any part of the stream. The water was clear and I observed active aquatic wildlife, such as minnows and crayfish. The clean-up is complete and I notified Terry Whitehair with Emergency Response.
1814440464 2017-07-14 Production Wells No Violations I was notified by Frank Riggle that their gathering line had been cut by the Township Trustee while cleaning out culverts. When I arrived at the site I found that the line had been cut with a back hoe in a small intermittent stream that connects with an un-named perennial stream. Rob Hunter had absorbent pads spread throughout the small stream. I did not find any observable crude oil in the perennial stream, but there was a slight sheen. I estimated the release to be less than five gallons based on the amount of oil I observed in the water and the surrounding soil. I instructed Frank and Rob to install absorbent booms at the line break site, at the point where the streams intersect and at a bridge on TR 21. I told them to keep working at the pads and leave the booms in place, and I would be back to follow up on Monday morning. The Ohio EPA was notified by the DOGRM Emergency Response duty officer.
