On the day of my inspection Randy Moran pulled the rods and tubing from the well. They found that the fluid level was down approximately 700. I instructed them to tag bottom with their tools before washing any clay down the well.

On the day of my inspection I found that the well was still sitting idle with rods and tubing. The tank battery has been diked and there was peat moss spread over the area of contamination. I called Barry Dugan again. This time he claimed that Randy Moran was moving in to plug the well on 4/17/14. I made it clear to Mr. Dugan that this was his last warning. The next step would be referral for a Chiefs Order. I also advised him that Clark Clay was not approved for the plugging of an oil and gas well at this time.

1335411343 2014-04-24 Plug / Plug Back No Violations FOLLOW UP TO CA# 1366911327: This inspection correlates with complaint # 4300. On the day of my inspection Randy Moran pulled 440 of 8.625 casing from the well. Then they washed 13 tons of clay into the well and let it settle overnight.
1366911327 2013-04-25 Production Wells Violations Noted I received a call from Mr. Reuben Byler in regards to an idle well on his property. On the day of my inspection I found that the well head was equipped for a pumping unit. The pump had been removed, the well was overgrown with multiflora rose, and the rods were extending from the tubing. The tank battery consisted of one 100 barrel stock tank and one separator, with a poly brine tank sitting down hill from the main components. The tank battery was in heavy vegetation, there was no dike, the identification was barely legible and there was a 2 ft x 3 ft pool of standing crude oil at the base of the stock tank. The standing oil and contaminated soil must be cleaned up and legible identification must be posted. In its current state this well is incapable of production. Make well capable of commercial production within 30 days; or plugging is required within 90 days. Failure to comply may result in forfeiture of bond.
1389901389 2014-01-16 Administrative Inspection No Violations FOLLOW UP TO CA# 1366911327: This inspection correlates with complaint # 4300. The well was transferred to Dugan Drilling on 7/16/13, they obtained a plugging permit on 10/18/13. I contacted Barry Dugan to inquire as to when they planned to plug the well. He claims he will try to get it done in the next couple of weeks.

On the day of my inspection Randy Moran ran his ripping tools in to a depth of 3000, then they pulled up to find a telegraph. They got the 4.5 casing parted at 2664 and washed 2 tons of clay down the casing. They pulled 20 joints and shut down to let the clay settle.

On the day of my inspection I found that the fluid level was approximately 200 down. I contacted Barry Dugan to request that they top it off with more clay and leave the well open over the weekend.

I received notification from Barry Dugan that they washed another 6 tons of clay into the well. He claimed that the well was filled to surface. I requested that he leave the well open for another 24 hours to let the clay settle.
1399040928 2014-05-01 Plug / Plug Back No Violations FOLLOW UP TO CA# 1366911327: This inspection correlates with complaint # 4300. On the day of my inspection I found that the clay was sitting firm at the surface. This well is plugged and abandoned. The area of contamination at the tank battery was cleaned up. This violation is resolved.
1430400518 2015-04-30 Final Restoration No Violations On the day of my inspection I found that the well site, lease road and tank battery site were never graded and seeded. The final restoration on this site was due on 11/01/2014. I called Barry Dugan about the restoration. He claimed that he had a signed landowner waiver. He met me in the field with a copy of the waiver.
