0141407146 2012-09-17 Production Wells No Violations I received a letter from The White Law Office on behalf of client Chip Bumpus requesting an inspection of a suspected idle well. On the day of my inspection I found that the well head was equipped with a pumping unit that was in a state of good repair. The lines were connected, the valves were open and there was oil/paraffin buildup on the stuffing box. The tank battery consisted of two 100 barrel stock tanks, a separator and an earthen dike. This well shares a tank battery with permit 4297. There was no identification posted at the well head or tank battery. I called John Mason (330-763-0436) to request that proper identification be posted at the well head and tank battery.
0205084730 2014-09-24 Plug / Plug Back No Violations On the day of my inspection Mason Drilling ripped the 5.5 casing at 2593. The clutch went out on the rig after they pulled one joint of the casing.
0466628403 2015-08-28 Final Restoration No Violations On the day of my inspection I found that the equipment had been removed, the site had been graded out and was growing with tall grasses and broadleaf vegetation. I found no erosion issues. The lease road was shared with permit 4297 and was still in place I called John Mason. He claimed that they already had a signed landowner waiver that had been submitted to central office to leave the road in place. I contacted his secretary and requested that she email me a copy. I am waiting on confirmation of the waiver.

1261654216 2016-10-12 Final Restoration No Violations On the day of my inspection I escorted surveyor Tom Smith to the plugged well site in order to mark the location with a flagged stake at the final recorded GPS coordinates. I found that the vegetation was well established and there were no signs of erosion. There was no evidence that the plugged well was leaking.
1411391952 2014-09-22 Plug / Plug Back No Violations On the day of my inspection Cemenco pumped a bottom plug inside of 5.5 casing. They set the plug through 1.5 tubing.
1412362830 2014-10-03 Plug / Plug Back No Violations On the day of my inspection I found that Berry Well Service had rigged up and pulled the 5.5 casing from the well. They had 1.5 tubing ran to a depth of 2239. Cemenco pumped cement plugs at the top of the Big Lime, through the Berea, at the base of the surface casing and the surface plug. Circulation was established and maintained through all stages of the plug job. Cement was circulated to surface on the last plug.
