0679740946 2013-08-19 Preliminary Restoration No Violations On the day of my inspection I found that the location was graded and planted with well established grassland vegetation. All drilling equipment and debris had been removed from the site. The well head was equipped with a pumping unit. There were no leaks or spills and there were no signs of erosion. This site passes preliminary restoration.
1355771574 2012-12-17 Drill / Deepen / Reopen No Violations On 12/14/12 H&D Drilling reported that the hole was caving in while running 10 drive pipe, so they pulled the casing and reamed the hole to 25 with a 12 bit. They were then able to drive the casing in to 58. On the day of my inspection I witnessed the 25 of annulus being cemented to surface with 2.5 yards of ready-mix cement.
1356015074 2012-12-19 Drill / Deepen / Reopen No Violations On the day of my inspection H&D Drilling ran new 8.625 casing to 423 with three centralizers. Formation Cementing caught up with the fluid, which was at 30 in the casing. Then they pumped 500 lbs of gel and cotton seed hulls, displaced out of the casing, and shut down for 15 minutes. After the break they pumped another 100 lbs of gel and cotton seed hulls followed by 140 sacks of cement. Circulation was established while pumping the second gel/cotton seed slug. The cement was circulated to surface but fell slowly after pumping stopped. Formation landed the plug and increased the pressure to 500 psi. The pressure fell by 200 psi in 5 minutes. I suggested letting the cement set up and re-testing the next day.
1356962903 2012-12-31 Drill / Deepen / Reopen No Violations On the day of my inspection H&D Drilling was not drilling. The well was drilled to total depth and production casing was cemented. The drilling rig was still rigged up on location and the permit was on display. There were no leaks or spills and there were no sags or tears in the pit liner. The well head was shut in. I contacted Formation Cementing to have the cement job log faxed to me.
2007413947 2012-12-20 Drill / Deepen / Reopen No Violations On the day of my inspection H&D Drilling used a 7 to 2 swedge equipped with a shut of valve and pressure gauge to conduct the casing test. They pressured up to 300 psi at 08:45. At 09:15 the pressure had decreased by 15 psi. I cleared H&D Drilling to resume drilling operations. We ran a weighted section of twine down the annulus and determined that the cement top was at 20. I authorized H&D Drilling to top it off with sack concrete.
