The field has been plowed and planted around the well, and the actual well site was covered with weeds. There were no signs of erosion. The well was equipped with a swage and ball valve. The valve was shut and there were no leaks. There was signs of excavation from laying electric and flow line to the well. I called and left a message with James Miller. I have had previous discussions with Mr. Miller where I emphasized that the four remaining wells on the Layman lease must be equipped and put into production this year. This well was drilled to total depth, and has been shut in since June 2014.

The motor, drive belts and electric were intact. The flow line was connected at the wellhead and the valve was open. Correct, legible identification was posted on the electric box. The production equipment consisted of two 100 barrel steel tanks in an earthen dike that are shared by the Layman 3 through 13 wells. Correct, legible identification was posted at the tanks for all of the wells on the lease. There were no leaks or spills around the wellhead or tanks. The well site was graded out and covered in vegetation. There were no signs of erosion and there were no materials or debris left behind. The preliminary restoration for this well is complete. The well status should be changed to producing (PR).

He reported that he spudded the well on 12/28/13, at which time he set 35 of 8.625 conductor casing. The permit was on location and there were no sags or tears in the pit liner. I informed Vernon that he is required to provide 24 hours notification prior to commencing drilling a new well. I also contacted James Miller to inform him that they were in violation for failure to notify, and I reminded him of his responsibility to provide such notification in the future.

The site was very muddy, and the sloping topography would make it extremely difficult to get vehicles or equipment back to the location. This is the final well on the Layman lease that needs to be equipped for production.
