The frack tanks have been moved off the site. The natural gas powered pumping unit was in operation at the time of my inspection. There were no fluid or gas leaks from the wellhead. The well is being produced into a common tank battery on the west side of TR 396. There was no identification posted at the pumping unit or tanks. |

On the day of my inspection I found that the rig was up and they were ready to spud the well. The drilling pit had not been dug yet and there was no casing on the racks. Wilsons Water was on site delivering fresh water for drilling. Dwayne Dugan met me at the site with the drilling permit. During permit review I noticed that the casing program called for the conductor casing to be cemented, even though the drillers intended to drive the conductor. I also notice the surface casing diameter was listed as 7.5 inch. The actual outer diameter of the casing is 7 inches. I called DOGRM Permitting Geologist Steve Opritza to ask about the discrepancies. Steve said that they could drive the conductor and he agreed that the casing size should be 7 inches.. He said that he would send me an email granting permission to deviate from the proposed casing program. Dwayne brought in 80 feet of new 10.75 inch diameter casing. I witnessed Max Donaker digging out the drilling pit with a bulldozer. The bottom of the pit was adequately smooth. There were no jagged rocks or tree roots present. The side walls were rocky, but there were no jagged rocks or tree roots. |

The perimeters of the access road were still rough bare soil mounds and downed trees. |

The frac tanks were still staged up at the drill site. The well was equipped with a natural gas powered pumping unit that was in operation during my inspection. I found an area along the access road where there was straw laid down and crude oil spray covering the soil berms of the access road. There were some small patches of thick, black crude oil in the vegetation along the side of the road. When I got to the top of the hill I found another area where they had a line dug up and patched together. There was approximately one barrel or less of thick, black crude oil spread across the ground in a 20 foot diameter. Finally, I found straw laid down around the wellhead. I called Travis Dugan to ask what happened and why I was not notified. Travis said that they were still flowing back some fresh water from the frac and the line froze. He said that they discovered the leak on 12/14/17. Travis did not have an answer on why there was no notification provided. I told Travis that I would like them to have everything cleaned up by the end of this week. Dwayne Dugan called me the following morning to notify me that they were in route to the site to clean up the oil and bury a flow line for the well. |

They had problems with the bit, and the air compressor, and the soap pump. The were able to start drilling out the plug with the rotary tools on air. There were no sags in the pit liner and there were no leaks around the drilling rig. Harold Donaker gave me the receipt from the sack cement used to fill in the spaces around the conductor casing. Derrick Petroleum has stone delivered and spread across the lease road. The 4.5 inch casing that was used in the Moore 5A was on a trailer at the bottom of the hill. I contacted Southeast Regional Supervisor Brent Bear to clarify the testing requirements for the casing, as it was pulled out of the Moore 5A after only four weeks of production. Brent confirmed that the casing will need to be pressure tested and drifted again, due to the fact that it was subjected to frac job and cement job pressures. |

I had spoken with Dwayne Dugan earlier in the day to notify him that the casing must be tested again before he could run it in this well. I explained to the reasoning to Dwayne when he protested. He asked that I meet with him at the drill site so he could physically see the rule that specifically required the casing to be re-tested. Travis showed up in Dwaynes place, so I read OAC 1501:9-1-08 (D) with him. Section D covers casing standards and specifically, reconditioned casing. I told Travis that they would either have to re-test the casing from the Moore 5A or bring in another string of new or tested used casing. Travis made a few phone calls and then left the site without further discussion. Travis called me back while I was still on location writing this report and said that he agreed with me, and that he would like to run the casing that they recently retrieved from the Moran plug job. |

There was a small amount of oil laying in the bottom of the hole, but the soil was mostly clean. The oil impacted soil that had been treated was spread out and there was no tarp. I didnt find any oil soaking up through the snow. The area around the wellhead was clean. The five frac tanks were still up at the well site. I did not find any leaks around the tanks, and I found no new leaks from the well or flow lines. |

They established circulation and started mixing cement. Circulation was maintained once it was established, with approximately 6 barrels of cement returns. Formation pressured up to 600 psi and shut it in for 5 minutes, with no pressure lost. |

Dwayne said that the casing pressure testing paper work was on the drilling rig with the well records. I went back up the hill and found paper work from January, 2017 labeled for the Moore 5A. I told Harold, Max and Mark Donaker that they can not run that casing until I see current documentation that it was re-tested since being removed from the Moore 5A. I called Dwayne and left a message stating the same thing. Dwayne returned my call and said that he had all of the papers in one envelope and he mistakenly left the old one. He assured me that he had a new document from Buckeye that was dated 10/27/17. I arranged to met with Dwayne later in the day to get the test document before H&D could run any pipe. Dwayne met me and produced a current document from Buckeye showing the casing was just tested. |
