0081957422 2013-08-05 UrbanProduction Wells Violations Noted The producing unit is comprised of a surface casing completion and a valve swage connected to a flow line. The wellhead is enclosed in a chain-link enclosure. Identification is present. I did not observe any evidence of leakage. The tank battery, which is shared with the #3 unit, is located on Eagle Valley Ct. of Harris Road. The tank battery is comprised of 2 100-barrel production above-ground storage tanks (ASTs) and one 50-barrel brine AST in a chain-link enclosure. The tank battery and access road have been rendered inaccessible by overgrowth of cattails and phragmites. The interior of the tank battery is overgrown completely. In the event of a fire or other emergency, response crews and equipment would not be able to access the tank battery.
0823984037 2017-09-14 Plug / Plug Back No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: overgrown vegetation at the battery and no accessible lease road. At the time of this inspection, the well had been plugged and abandoned and the battery removed and the area restored. No further action needed.

1768687220 2014-12-12 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead is located on the northeast side of the Family Heritage Life Insurance facility located at 6001 East Royalton Road. The wellhead is comprised of a mechanical plunger lift system and is enclosed in a small chain-link enclosure. Fittings and valve connections at the wellhead are secure and free from leakage or problems. The tank battery is located on Eagle Valley Court on the west side of a large berm created during site development. The battery is comprised of 2 100-barrel steel tanks and separator unit. The batery is shared with the Dashner #3 well located on the west side of the Family Life Insurance building. At the time of this inspection there is no identification at the battery. The interior areas have become overgrown with vegetation. I contacted Dave Mansbury with Duck Creek Energy and he indicated that maintenance and signage issues will be addressed ASAP.
1795603224 2017-12-01 Final Restoration No Violations I inspected this former well location for final restoration requirements. Plugging and abandonment were executed and completed on 7/28/15. The plugging activity was witnessed by an ODNR inspector and a plugging report was filed. At the time of this inspection, the well location as been restored and developed for commercial use. No elements of the production system remain. The flow-lines were removed. No further action is needed. Final Restoration is passed.
1871761677 2017-10-20 Final Restoration No Violations I inspected this well location as an evaluation for Final Restoration. Plugging and abandonment was completed on 7/28/2015, witnessed by a ODNR Division of Oil and Gas Inspector, and a plugging and abandonment report was filed. At the time of this inspection, the former location of the well had been regraded, and redeveloped for commercial use. The former battery location had been restored. The former production elements, including the flow-line, were removed and there was no remaining system elements. No further action is needed. Final Restoration is passed.
