0321691093 2015-04-02 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The well and tank battery are located in an open behind the house at the referenced address. The well is comprised of a pump jack system in a chain-link enclosure. The battery is comprised of 1 100-barrel steel production tank, 1 100-barrel poly brine tank, and mechanical separator. At the time of this inspection I observed some free-floating oil within the battery containment. Some mitigation efforts, as evidenced by the presence of several dozen absorbent pads and several booms, have been performed. I did not see any evidence of oil outside of the containment. Otherwise, fittings and connections at the wellhead and battery are secure. Load-outs are secure. Gates are locked. Process safety equipment is present. The access road is good, passable condition with no washouts or other erosional deficits or signs of impact. I contacted Rick Reeve with Gonzoil regarding the oil in the containment. He indicated that the leak had been discovered after the snow melt. He said the remaining oil is being cleaned up and mitigated as it appears during rainfall events.
0679493091 2015-07-07 UrbanProduction Wells No Violations I inspected this producing unit as a follow up to an earlier inspection. At the previous inspection, I had observed a oil in the tank battery from an apparent release. Gonzoil Inc. indicated that efforts to mitigate and correct the problem were under way. At the time of this inspection, all of the free oil has been removed, but some oil-stained gravel ballast material remains. I contacted Gonzoil Inc. Doug Gonzalez indicated that they had been waiting for the rains to subside and allow clean-up to be completed. He indicated that this would be completed within two weeks. The unit otherwise is secure and free from signs of leakage or problems. Gates are locked and secure. Load-lines are plugged and locked. Hatch lids are sealed.

1043016056 2017-05-24 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The wellhead is located in a small clearing in a wooded portion of a parcel on Akins Road utilized otherwise for agricultural purposes. At the time of this inspection, I observed the following: Fittings and connections at the wellhead and battery were secure and free from leakage. The load-line valves were closed and plugged but missing locks. Tank lids were closed and secure. Individual enclosure gates were locked and secure. I inspected both the interior areas and exterior portions of the well and battery and surrounding areas and found no signs of impact or problem.

The production wellhead is fitted with a pumpjack system and is currently producing. The pumpjack unit is enclosed in an 8-foot chain-link enclosure which is locked. The annular space is plumbed back to the production line. The tank battery consists of 2 100-barrel steel tanks, 1 50-barrel steel tank, and separator. The battery is enclosed in a chain-link enclosure. Safety equipment (carbon filter, spark arrestor, low-pressure relief valve) is present. The secondary containment is present and in good condition. No violations noted.
1431542806 2015-05-06 UrbanProduction Wells No Violations I was on site to perform an inspection with DOGRM Inspector Shannon Mullins. The tank battery and pumping unit are enclosed with a chain link privacy fence. Upon our inspection we found that the tank hatch on the two 140 barrel tank were equipped with the proper sealed hatch and lightning arrestors are installed. There is a charcoal filter installed with a vent. There were locks on the water and oil pick up line. All drums needs to be removed. The tank battery entrance gate was locked. The wellhead gate was locked. The well is equipped with a pumping unit and an electric motor. There is oil staining on the ground at the wellhead. The electric box was not locked. ID signs are posted. DOGRM Inspector Steve Tompkins had inspected this location on 4/2/2015 and noticed an oil release. He had contacted Rick Reeves about the clean up. At this inspection there is still some clean up that needs to done around the inside of the dike area.

There were several inches of snow in the area.

The oil had been related to a release that had occurred within the battery and was successfully mitigated but some residual material remained. At the time of this inspection, the owner has completed the mitigation and clean-up. Fittings and connections at the wellhead and battery are secure. Load-lines and electrical panels are locked. The interior dike is in good condition with a some ponded storm water which is clear and free of oils or sheen. The areas surrounding the well and battery are comprised of pasture land and gravel drive. A reconnaissance of these areas do not reveal any signs of impact associated with the producing unit. The dedicated unpaved access road is well drained. The gate at the head of the access is locked and secure. ID signage is posted at the road.
