The well and battery are located in a wooded area with a dedicated, unpaved access road with gated entry. The access road was in a good condition and passable. The enclosure gates were locked. The well enclosure which had been partially missing at my last inspection, had been repaired and was secure and locked. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. There was an active chart for the Nero well and for the Lisy (2-2174), which shares the battery. I walked the areas surrounding the well and found no signs of problems or impacts relating to the production operation.

1937905652 2014-07-16 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 11/03/2008. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and pump jack system. The wellhead is located down a dedicated access road which enters off of Dewey Road. The access is gravelled and portions are paved with re-claimed asphalt material. The road is in reasonable condition. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located near the head of the road. The battery is comprised of a total of 2 steel 140-barrel production tanks, 1 140-barrel steel brine tank, and horizontal separator, and is shared with the #2-2174 unit . Spacing from well to battery, and spacing of elements within the battery meet regulations. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition.
2056379613 2017-03-16 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger lift system and associated tank battery located in a clearing within an otherwise wooded area. The wellhead and battery were in separate board-on-board enclosures which were locked. I noted that a portion of the wellhead enclosure had fallen down. Access to the battery and wellhead was afforded by a gravel-paved access road which was snow-covered, but accessible. The battery was comprised of two 140-barrel steel production tanks, one 140-barrel steel brine tank, and horizontal separators. The battery is shared with the #2-2174 Lisy #1. The fittings and connections at the wellhead and battery were secure and free from signs of leakage. The load-lines were plugged, locked, and secure. The ID signage was present and accurate. Process safety equipment was present and in good condition. I notified Duck Creek about the wellhead enclosure needed maintenance.
