1056554298 2003-06-25 Plug / Plug Back No Violations -Witness second stage of plugging operation. Met with Bob Huwer of Huwer Drilling Co. Bottom stage of the hole plugged 5/24/03. 3 yards of 9 sack grout were used on 5/24/03.

-During inspection on 6/25/03, Huwer used additional 2 3/4 yards of 9 sack grout. -Huwer planning to pull the 4.5 inch diameter casing and finish plugging with Class A cement within the next week. Bob Huwer to contact inspector when he plans to finish the work so inspector can be present. -8 inch and 6 inch casing both full of holes/rotten. Two small 1-1.5 foot long pieces is all the operator was able to recover.

1062004333 2003-08-26 Final Restoration No Violations -Final Restoration Inspection. Bob Huwer of Huwer Drilling Co. completed plugging the well on 8/15/03. After witnessing the plugging, Huwer cut off the surface casing pipe at least 2-2.5 feet below surface and added well ID plate. Hole dug to cut surface casing filled back in. All pipe removed. Service rig removed. No circulation pit dug so no restoration. Well in edge of corn field and will be planted with crops in 2004. Regular photos taken of the restored area.
