The storage consist of one silver 100 barrel tank #3277 and a 1000 gallon plastic brine tank. No identification or emergency contact information is posted at the well or storage. Follow-up inspection (4/5/13) on the Innis #1 (#104) noted additional violations. An unlined earthen pit is being used for brine disposal through evaporation. The pit measures 21W x 31L x 1.5D and is filled with fluid which measured 1264 ppm Chlorides. The following actions need to be completed to bring the Innis #1 into compliance: 1) The oil contamination and brine pit need clean-up and remediated as soon as possible. 2) The well identification and emergency contact information needs legibly displayed. 3) The current owner needs to register and maintain insurance with the State of Ohio. 4) Transfer of ownership forms need completed and statement of production needs filed if applicable. 5) Or in lieu of producing the well owner may receive temporary ownership status, receive a valid plugging permit, plug the well and restore/remediate the site to its original condition.
