An inspection of tank battery location revealed parted 2 orange plastic line over hillside from bottom of 200 bbl horizontal saltwater tank. The bottom valve on this tank was closed at time of inspection. An area measuring approx 200 x 5-20 contained stressed vegetation extending from this line to creek. Patches of vegetation were oberved and will require remediation. The impacted area did not appear to be new. Traced remaining section of line to injection well, Permit 160. At 11:38 am, the well owner representative, Mr. Bill Jackson, 614-570-4318, was notified of inspection results. Mr. Jackson states that spill occurred last year and cleanup was witnessed by Sam Faust. Mr. Jackson agreed to remediate impacted area. Note: till, organic material, lime, seed, mulch. The plastic line will also be reconnected.

0309376156 2011-05-27 Production Wells No Violations The Division received a complaint alleging that oil was running down a driveway from this well location. An inspection of the gravel driveway which measured approx 750 showed no sign of crude oil. Arrived at the well location to observe a service rig owned by Fishburn running rods back into the wellbore. The rig was placed on the well to replace a joint of tubing which had contained a hole. A 5x5 area at wellhead contained crude oil and approx 1 x 5 area near front of rig. An area next to well location contained oil stained grass from approx 2 joints of tubing. Puddles of water along the lease road located near tank battery (note: higher elevation than ) showed iron scales and not crude oil. Verified wooded area below well location and found no crude oil impact. Interviewed well owner and representative at the location. The well owner states no impact offsite and further stated OEPA responded earlier this morning. The service rig company will remove contaminated soil at 1:00 pm. Notified the complainant of inspection results at 11:46am. The complainant stated that he may have jumped the gun but had concern with runoff from a rainfall event. At 12:20 pm, Mr. Wes Drake with OEPA was contacted to discuss inspection results. A follow-up inspection will be conducted.

A new tank will be installed Thursday or Friday of this week. The saltwater line has been fused together. They plan to establish vegetation this fall for impacted area from last years brine spill.

1846811498 2011-08-24 Production Wells Violations Noted The Division received a landowner complaint alleging that a brine spill had occurred since the last inspection. Mr. Bill Jackson, the well owner representative, arrived at well location prior to the inspection of the well site. Mr. Jackson states: They discovered two holes in the saltwater tank and have been repaired; Brine should have been contained inside the dike; and saltwater line routed to nearby injection had been fused together; He feels there could have been brine released from this line because, saltwater was recently transferred through this line. Mr. Jackson would not stay for the inspection. An inspection revealed two impacted areas. The first area derives from the fused saltwater line located behind the tank battery. This area measured 195 x 10. The path leads to an intermittent stream. No evidence of crude oil found. Note: Previous inspection had shown stressed vegetation from brine spill last year. Pockets of fluid were tested near source and approx 150 away with High Range Quantabs and showed 8.4 and 6.6 units respectively. The second impacted area was located on backside of earthen dike and measured approx 40 x 20. The source for this area was from the now repaired saltwater tank. Two ground hog holes were found inside the dike, which may have allowed saltwater to be released from dike. At 3:59 PM, Mr. Jackson was verbally notified by telephone today and instructed to remove all contaminated material and address the earthen dike within 1 month.
