0615036417 2013-01-10 UrbanProduction Wells Violations Noted While the signs for the wells were adequate for production wells, under the urban rules the signs must include more information. I have provided the information necessary, according to 1501:9-9-05E1, to compete this task. There must be signs at the gate entrances on the access road, wellheads and tank battery. Fencing is missing around the tank battery which consists of two 100-barrel tanks, one for oil production and the other for brine water separated from the oil in a small knock out tank on top of the brine tank, a gas-fluid separator, small knock out gas-fluid separator and a gas meter. Tanks must be checked to ensure they are in compliance with 1501:9-9-05E4. Requirements for the fencing are found under 1501:9-9-05E2 including locks, according 1501:9-9-05E3.
1199888434 2013-06-10 Administrative Inspection No Violations Jack Chamberlain of Old South Oil, LLC, contacted me on clarification of urban rules for his three wells in Gallipolis Township, the Glassburn #1, the James Haskins #1 (API 3405320210), and the Bastiana #1 (API 3405320299). More specifically we discussed identification signs and fencing around the tank batteries and wellheads and identification signs on access road gates to the well sites. I have already provided identification signs used in urban areas located in Lawrence County.
1357908656 2013-01-10 Production Wells No Violations Wells pumping unit resting on precast concrete forms. Well has identification sign with all the relevant information. At time of inspection all valves were closed. Wellhead tree has horizontal pipes attached for gas and oil. No CH4 (methane) gas detected at the wellhead using the Honeywell PhD6 gas meter. Tank battery consists of two 100-barrel tanks, one for brine and the other for oil. Well identifications for this well and the Bastiana #1 are displayed on the oil tank. There is an oil/water separator resting on top of the water tank. The gas goes through a fluid separator and a knockout cylinder before going through a gas meter. Checked graph recorder and presently no gas production was recorded. Attached to the gas line is a one inch black plastic line that leaves the site and goes in the direction of the other well supplying product to this tank battery, Old South Oil, Bastiana #1. The plastic line goes through the dyke that surrounds the tanks and separators.
1906590026 2013-03-19 Administrative Inspection No Violations This is one of the wells on D. Goins Special Project list for March/April. Original compliance notice was issued by myself on 01/23/2013, # -615036417. Access road GPS location is at the gate on the east side of the property.
