0014319699 | 2011-05-10 | Production Wells | No Violations | Company contacted me in regards to an oil leak in side the dike. Initial estimates of oil spilled into the dike is 55 BBl of oil. At the time of arrival operator was removing remaining oil from the tank. Operator had removed the majority of the oil from the battery before arrival. Valve on the west side of the tank is bent from the tank settling into the wet clay. Operator will, prevent further escape of oil from the tank, remove oil from the damaged 100Bbl tank. Remove remaining oil in the dike. Strip tank battery, removing damaged tank, remove grossly contaminated soil and gravel from the tank battery. Take grossly contaminated soil to an appropriate landfill, replace stone in the tank battery and replace damaged tank. No oil escaped the tank battery, walked area around tank battery and well site no drain tiles noted, no oil noted in the pond east of the tank battery down slope of the battery and no oil noted on ground around outside of the battery. Tank battery has a three strand barbed wire fence around it to protect livestock. Identification sign was posted on the tank. Wellhead was in good repair. Spacing was correct in regards to separator, well and tank. Tank had 72 inches of oil in it before spill. After pumping remaining oil from the tank 12 inches of sludge was left in the bottom of tank. Sludge couldnt not be pumped out. Operator will schedule steam cleaning of the tank before removing from location. |
0755230371 | 2014-11-20 | Production Wells | No Violations | My inspection found the well to be equipped with a plunger lift unit. The well and tank battery are in good repair. The tank battery consists of a 100 BBL production tank and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition. |
1305156972 | 2011-05-11 | Production Wells | No Violations | Clean up of oil spill on 5/10/11. Tank battery contained 1 100BBL production tank and mechanical seperator. Pools of oil present throughout the dike, with the largest concentration at the base of the 100BBL tank. Enervest had pumped the majority of the oil out of the dike on 5/10/11. All oil contained in the dike, no evidence of dike being breached. Scott Miller arrived on-site at 9:00AM and began scraping oil soaked gravel and soil from the tank battery. The saturated material was trucked to Enervests yard in Garretsville. Jerry from Enervest arrived and pumped out the remaining puddles of oil still within the dike. Once scraped, a load of new gravel was spread in tank battery. While attempting to slide production tank closer to seperator, 2 holes were discovered in the bottom of the tank. Scott Miller placed the tank on its side to stop any leakage and diked the tank on all sides. The tank will be steam cleaned and replaced with a new tank on 5/16/11. Upon departure, tank battery was completely diked in with peat moss covering all remaining oil. Off-site at 12:45PM. |
1305638111 | 2011-05-17 | Production Wells | No Violations | New 100 BBL tank in place and connected to seperator, not yet in production. Excavated portion of dike is seeded and covered in straw. Joe from Enervest stated a small amount of oil had breached the dike and accumulated in an old foundation downslope of the tank battery due to excesive rain over the weekend. The sheen was boomed and diapered to the landowners approval, according to Joe. A temporary dike was installed around a large puddle of water outside othe tank battery and a siphon dam installed to prevent further movement of oil and water. A ditch was dug on the right side of lease road to prevent runoff from leaving the site. Joe stated he would remove the temporary dike after the puddle dries and weather permits. He also stated the lease road would be resurfaced with gravel. |
1306243332 | 2011-05-24 | Production Wells | No Violations | Oil spill has been remediated, affected area has been graded and seeded. Tank battery has been reconstructed. 100bbl tank and a mechincal separator are in the battery. New barbed wire fence has been installed around battery. Operator has constructed a berm down slope of the battery with a siphon dam and boom in it to collect any residual oil. Dike around the tank battery has been built u and improved. |
