1300364554 2011-03-16 Production Wells No Violations Valve at the end of the load line from the production tank has a covering of oil and paraffin on it. There is paraffin on the ground under the valve approximately a half gallon. There is a black plastic line that comes off of the separator and runs across the lawn to the west approximately 100 feet. The line goes to a drip. There are two line connectors on the line. The first connector from the separator is leaking. There is oil residue on the grass in a 2 foot radius. Identification sign is from the previous operator. I contacted Tony pumper for Enervest (330)506-5909 and requested corrections to the identification sign and oil residue to be cleaned up. Tony, has a list of wells and identification signs on hand for wells that need them. He is waiting for the weather to warm, for the signs to stick.
1316526068 2011-09-19 Production Wells No Violations Checked the status of oil spill at the well. All free oil has been removed from location. Peat sorb has been mixed into soil. Affected area has been seeded and mulched.
1555557861 2011-09-16 Production Wells No Violations On site in regards to a reported release of oil. Operator contacted me to inform me that a stuffing box had failed and the well had released oil onto the ground. At the time of my arrival operator had free oil contained. Approximately 1/2 barrel (20 gallons) of crude oil had been released, affecting an area 25 X 60. Affected soil will be removed from site. Area will be bio-remediated, seeded and mulched. Operator removed the back pressure valve from the well, which had salted off and failed and caused the failure of the stuffing box. Repairs will be made to the well.
