0059376759 2011-12-12 Production Wells No Violations FOLLOW UP TO NOV # 1181254640 This well is equipped with a pumping unit. The bridal is off of the unit and the motor has been removed. Young trees and brush are growing up through and around the equipment. The tank battery is also overgrown and the backside of the dike has been breached. The well valves are closed and the flowline to the separator is closed. The chart on the gas meter shows no production. The DNR database shows no reported production since 2006 and records prior to 2006 shows declining production. The identification sign needs updated to show the current owner. The brush around the well head and tank battery needs to be removed. The well needs repaired as needed and put back into production or a plugging permit needs to be applied for and obtained by 02/05/2011. The well owner needs to comply with the terms of the plugging permit. Temporary Inactive status will not be approved for this well as there is a pending complaint associated with the status of the well.
0078901808 2013-10-02 Drill / Deepen / Reopen No Violations My inspection today found Chipco had made it through the surface plug. Upon drilling through the surface plug they encountered a small amount of oil. The bottom of the surface casing plug was tagged at 350. They are in the process of drilling through the second plug.
0109690702 2015-08-18 Final Restoration No Violations My inspection found that vegetation has been established on location. The well is showing no signs of leaking. Final restoration can be passed.
0129295790 2013-04-10 Final Restoration No Violations My inspection found a 5ft x 10 ft pool of oil on the surface near the location of the well. Once the area was saturated with water I observed a puddle of water that was bubbling. I notified Steve Shrok from Enervest about the location. He had an excavation company there to contain the oil on the ground. The area was absorbed with peat sorb and the contaminated soil was removed. The excavator then opened up the area where the plugged well was located. Enervest Supervisor Ellewood McDaniels and I witnessed the plugged well was leaking inside the casing around the cement plug. I instructed him that the well would have to be drilled out and replugged. The chain link fence also needed removed and the tank battery reclaimed.
0422223372 2012-09-10 Production Wells No Violations The wellhead is located approximately 500 feet east of Jug Road (CR 46). I observed the wellhead to be fitted with a pumpjack which appears to be inactive. The wellhead is completely overgrown with weeds and dense foliage. The tank battery (TB) is located approximately 300 feet to the southwest. The TB is comprised on one 210 barrel (BBL) production tank, one 110 BBL brine tank, and the remnants of a mechanical separator. The TB area in enclosed within a chainlink fence which is in a state of decay and disrepair. The interior of the TB is overgrown with foliage and small trees. This unit appears to have been idles for an extended amount of time. A follow-up with Tom Havranek of Enervest, indicates the unit is slated for plugging.

1181254640 2011-12-12 Production Wells Violations Noted Well is a pump jack well. Bridal is off of the jack. Motor has been removed. Young trees and brush are growing up through and around the equipment. Tank battery is overgrown. The tank battery dike has been breached on the backside. There has been no reported production per RBDMS since 2006. Records before 2006 shows declining production. Well valves are closed. Lines to the separator are closed. Chart in chart box shows no production. Identification sign needs updated to show current owner. Brush around the well head and tank battery needs to be removed. Well needs repaired as needed and put back into production or a plugging permit needs to be applied for and obtained by 02/05/2011. The terms of the plugging permit need to be complied with. Temporary inactive status will not be approved for this well, there is a pending complaint associated with the status of the well.
1308931623 2011-06-24 Production Wells No Violations Received call in regards to well being idle. No change in status of well since last inspection. Valves on the production casing are closed. Yoke is off of the horse head on the pump jack. There are no drive belts from the pump jack motor to the jack. Trees are established next the pump jack and behind the jack. Tank battery is over grown. Current identification sign has not been posted. Previous owners identification sign is still posted. No production reported since 2006.
1366121526 2013-04-10 Final Restoration No Violations On site with Kevin Perry, Geauga County Inspector, this well has been plugged. In the area of the work pit, there was standing water with crude oil floating on it. I observed the pit; it appeared that there were bubbles coming up through the water. I instructed Kevin to have the oil removed, pit material removed and the well exposed to check the integrity of the plug job. Kevin Perry contacted me that night; stated Enervest constructed a dike around the well. It appears the well is leaking and will need remedial action. Tank battery needs removed and graded and well needs repaired before Final Restoration can be approved. Complaince Notice to plug the well is closed.
1380111665 2013-09-25 Final Restoration No Violations My inspection today found the chain link fence around the tank battery has been removed. The sales lines and flow lines have been removed from inside the tank battery. The area has been graded, seeded and strawed.
1380642502 2013-10-01 Plug / Plug Back No Violations My inspection found Chipco on location to drill out the top two plugs of the well. The pit is in stable condition and the liner is in good condition. They are in the process of drilling out the surface plug.
1380890216 2013-10-03 Drill / Deepen / Reopen No Violations I was on location today to check on the status of the rig. When the rig crew arrived this morning they informed me that the well was not showing any more signs of leaking. I witnessed them drill out the second plug. The plug was from 350 to 550. After they made it through the second plug there was no signs of oil or gas leaking from the well.
1380897229 2013-10-04 Drill / Deepen / Reopen No Violations I arrived on location as Chipco was tripping the drill pipe out of the hole. They then sent a tool down hole to tag the top of the big lime plug. The lime plug was tagged at 2118. They are going to trip tubing down hole to set the first plug on top of the big lime plug.
1381167345 2013-10-07 Plug / Plug Back No Violations I was on location today with Inspector Rick Keppler for the plugging of the well. Chipco had run tubing down the hole to set the plug on top of the previous Big Lime plug. Omega was the cement contractor on location to perform the job. Omega broke circulation and pumped 60 sacks of Class A cement. The cement was displaced to 1946. Chipco pulled the tubing to 619. Omega rigged up, broke circulation, and pumped 65 sacks of Class A cement to cover the bottom of the 8 5/8 casing. The cement was displaced to 419. Chipco pulled the tubing up to 228. Omega rigged up, broke circulation, and pumped 70 sacks of Class A cement to the surface. Cement returns were witnessed. After the tubing was removed from the surface, the hole was topped off with cement.
1381234338 2013-10-08 Plug / Plug Back No Violations My inspection today found the well still exposed. The well is no longer showing signs of leaking. The pit is still open and Chipco equipment is still on location. When the area is graded and vegetation is established I will pass final restoration.

The riser pipes are still plumbed above ground in the tank battery. Still waiting on Enervest to drill out the plugs and replug the well.
1935704994 2011-03-21 Production Wells No Violations Well is a pump jack well. Valves on the well are closed. Yoke for the sucker rods is off of the pump. Rods are oxidized. Chart doesnt have a date on it and is smeared from moisture. Production records show no production since 2006.
2036442338 2013-10-07 Plug / Plug Back No Violations I was on location today with Rick Keppler to witness the replugging of this well. Chipco was the service rig operator on location and Omega was the cement contractor. Chipco ran tubing down the well to 2146. Omega broke circulation and began pumping 60 sacks of Class A Cement to set a 200 plug on top of the previous Big Lime plug. Chipco pulled the tubing up to 619 for the next plug. Omega rigged up and broke circulation and began pumping 65 sacks of Class A Cement to cover the bottom of the surface casing. Chipco pulled the tubing up to 228. Omega rigged up and broke circulation and began pumping 70 sacks of Class A Cement. Cement circulated to the surface. Chpico removed the tubing from the hole. The hole was then topped off with cement. GPS coordinates are +/-7.
2096758108 2013-09-05 Final Restoration No Violations I was on location today with Tom Hill and Mr. McDaniel from Enervest. Enervest wanted to dig the plugged well up again to reassess the well. The well was confirmed to still be leaking gas through the plugs. Enervest was advised to have the plugs drilled out and replug the well.
