0089316753 2018-01-02 Production Wells No Violations At the time of this inspection, the well was equipped with a plunger lift assembly and the annulus and flow line valves were open. Production records for this well were current. The tank battery consisted of one 210 BBL production tank and a mechanical separator. Valves on the separator were open and the chart showed production. The tank battery was diked and had legible signage. The lease access was stable. No violations observed.
0562428055 2008-10-06 Preliminary Restoration No Violations Well is a rabbit well. Site has been graded, seeded and mulched. Vegetation is established. Tank battery is in good repair. One 210BBL tank and one poly 1500 gallon tank, mechanical separator. Lease road is stable. Spacing is within 1509 limits. ID sign is up on tank battery.
0696788888 2008-04-25 Drill / Deepen / Reopen No Violations On site for surface cement. Surface hole TD at 485 11 inch hole. Operator ran 452 of 8.625 R-3 23 lb new pipe. Cement (.2542 X 452)/ 1.18 = 97.81 sacks X 1.5 = 150 sacks of cement needed for the job. Driller ran more than the 150 sacks needed. The driller (Cap Star Rig #1) had problems with circulation, at 120, operator lost circulation. Circulation was regained. Berea top 326 bottom 397. Operator was waiting for cement company. John Dixon was not circulating the well while he was waiting, he didnt want to lose his fluid to the hole. I instructed the cement company, Petroset and John not to run cement until circulation is gained. Operator broke circulation, then lost it, ran three bags of hauls and two bags of gel. Good circulation was gained. Operator ran cement, circulation was lost for two minutes when running cement. Circulation was lost when operator changed valves to send plug down. Circulation was regained, 4 bbl return of good cement to surface. Cement fell back once plug was down. Operator will need to tag cement and grout from the top as needed.

The tank battery consists of a 210 BBL production tank, a 35 BBL poly brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.

Lease road is stable and constructed out of durable material. Site is clean no fuel, oil or other leaks from equipment. BOP is on site. Conductor casing is on site. New 32 lb pipe. Operator will set at least one joint of CND.

Petroset mixed Gel on site, ran gel ahead. Mixed cement until it came around then displaced. Good return to surface of cement.

Gamma Ray Density ran. Top of the Lockport 3560. TD for the well is 4346. Petroset Cement company on site. Operator ran new R-3 9.5 long string pipe. Operator ran 200 sacks of 50/50 Mix with 2% Gel 10% salt. Weighed cement twice during job. First weight 14.6 lb/gal. Operator thinned the mix cement weight 14.4 lb/gal. 50/50 Mix with gel and salt weight is 14.39 lb gal, yield 1.29. Good circulation when displacing the plug. Petroset pressured up to 1500psi plug down 3:00pm. Checked scales with fresh water before job. Scales are correct.
