0013468652 2008-04-09 Administrative Inspection No Violations Spoke to Bob Randle at 7:00pm regarding the Conductor pipe that needs to be set. Bob said that the area is shallow till and he will be setting less than one joint of Conductor. I informed Bob that cementing less than one joint of CND can be waived. Spoke to Bob at 7:00am 04/10/2008. Bob said that they had drilled the offset wells in the area and had set less than 200 of surface casing on the wells. Bob also asked if he needed to set CND. I looked up off set wells in the area. Permit 1974 218 of SRF, Permit 1819 354 SRF, Permit 1885 305 of surface and Permit 1917 343 of surface. I spoke with Jay Cheslock DMRM supervisor regarding surface. We agreed that CND pipe can be waived. Surface Hole to be drilled on fresh water unless gas is encountered then they need to switch to mud. Surface Casing to be set per the permit.
0130049801 2010-07-15 Preliminary Restoration No Violations Well is a rabbit well. Board on board fencing is around well head and tank battery. Tank battery is 32 feet from a garage and over 100 feet from the home on the property. Lease road is stable. Site is stable. One 100BBL tank and 210BBL tank is on location. Mechanical separator outside of the dike is inside the fence. Identification sign is up.

Drainage coming out of the woods to the south needs diverted around site. Pits have been constructed and are in good repair. Lease road is stable and constructed out of asphalt grindings.

1374069721 2013-07-17 Production Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve is open. The well and tank battery are in good repair and are surrounde by a locked wooden fence. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
1802936406 2008-04-18 Drill / Deepen / Reopen No Violations Drill rig has moved offsite. Zero pressure on the annulus. Operator has a bull plug on the north side of annulus and a bull plug with a needle gauge on the south side of annulus. The trash pit and one blow pit are in good condition. The pit to the north west of the well head, the dozer operator has knocked the corner of the bank off. Lease road is stable. Called Tim Reilly from John D. Operator has cleaned up drainage ditches on both sides of the lease road. I asked Tim to put drainage controls at the discharge point of the ditches to prevent erosion. Spoke to time regarding pits, they are pumping them down. Operator will run bond log before they frac the well. 04/16/2008, operator called me before TD. Operator reported gas at 1000. Operator was not sure if the gas was above the 1000 mark or not. Gas was noted bubbling when driller made connection. Informed me that there were no issues with circulation. Hole went well. Operator stated that he was going to run 200 sacks of cement for long string. Operator ran geo-log before running long string.
2066774584 2016-03-21 Production Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valves is closed. There are multiple fittings leaking on the wellhead that need repaired. The well and tank battery are surrounded by a wooden fence. Locks need installed on the fence man doors. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The load line valves on the tanks have plugs in them but are not locked. The identification sign on the tank battery fence needs replaced. The lease road is in stable condition. I have contacted the company about the issues.
