0117534554 2017-07-20 Production Wells Violations Noted On the day of my inspection I witnessed the well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, rods and a Cabot pumping unit powered by electricity with 1 of 2 valves open on the tubing and 1 of 2 valves open on the casing. Also present near the pumping unit a compressor unit and a drip were located. Production equipment included 2-horizontal separators, a vertical separator and a drip. Also present 2-210 barrel tanks and a 50 barrel tank within an earthen dike. All equipment appears to be tied in and shared with 34059236340000 Morris-Palmer-Bichard 2. A 3 feet by 4 feet area of oil contamination was present near the 3 inch load valve on the south tank. Also within the dike a 4 feet by 20 feet area saturated with standing brine was found. Fluid was field tested and revealed a pH of 6 and chloride levels of > 6215 parts per million. Identification was present by not legible and production records were current. Owner/Operator shall properly identify well in accordance with OAC 1501:9-9-05 (A) (10). Owner/Operator shall remove fluids from dike, remove contaminated soils, replace with clean fill, dispose of properly in a solids landfill and provide ODNR with the manifest for the disposal.
0652703856 2017-09-07 Production Wells No Violations On the day of my inspection I following up on Compliance Notice 117534554, I witnessed the contamination has been removed and the dike remediated along with being rebuilt. Identification has been posted resolving the compliance notice.
1501085611 2017-07-26 Production Wells No Violations On the day of my inspection I following up on Compliance Notice 117534554, I met with Deer Creek personnel Dave Williams and Susan Bates to discuss the notice. The contamination in the dike needs to be removed, remediated, replaced with clean fill seeded and mulched. It was recommended that the 50 barrel be removed, discarded and the dike rebuilt. The contaminated soils need to be disposed of in an approved landfill and manifest need to be provided to ODNR upon disposal along with the well identification updated.
1861496909 2017-08-17 Production Wells No Violations On the day of my inspection I following up on Compliance Notice 117534554, I witnessed no action has been taken to resolve compliance notice with notice being due 8/18/17. I placed call to Susan Bates of Deer Creek and requested an update on when they were planning on beginning on requested action. Ms. Bates stated that they have an operator moving in and will begin on Monday.
