0114408921 2017-01-18 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a disconnected 210 barrel tank within an earthen dike. The well was fenced in and found equipped with 8.625 inch diameter casing; 4.5 inch diameter casing, 1.5 inch diameter tubing, a plunger lift system and domestic gas service with all valves in the closed position. Identification was present but faded and production records were current with zero production reported since 2010. I contacted Enervest requesting their plans for the well.
0202229524 2017-03-15 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Jimmy Wiles, I witnessed Chipco LLC Rig #6 was onsite and rigged up with Hydrogen Sulfide (H2S) being detected on the well at > 100 parts per million (ppm). Monitors were in place along with a ventilation fan. Appalachian Solvents Well Treating and Testing (ASWTT) was onsite and pumped 150 gallons of Sulfa-clear down the 1.5 inch diameter tubing pressuring up to 200 pounds per square inch (psi). ASWTT pumped an additional 450 gallons of Sulfa-clear down the 4.5 inch diameter casing. Chipco tripped inside the tubing and reached a depth of 5327 feet. Appalachian Well Surveys (AWS) was onsite and tripped inside the 1.5 inch diameter and shot the tubing off at 5310 feet. Chipco pumped a 30 sack 400 foot Class A Clinton plug from 5310 feet to 4910 feet. Cement was displaced with 12.3 barrels of fresh water. Chipco tripped the 1.5 inch diameter tubing out to 3770 feet and AWS tripped in and was unable to drop out of the tubing. AWS tripped out and rigged down. Chipco tripped remaining 1.5 inch diameter tubing from wellbore and will attempt to rip casing in the Big Lime tomorrow.

0532154749 2017-03-16 Plug / Plug Back No Violations On the day of my inspection I witnessed that Jim Green had hot tapped the 8.625 inch wellhead valve and no Hydrogen Sulfide (H2S) was detected. The 4.5 inch diameter casing was loaded with 80 barrels of brine. Chipco rigged up their ripping tool and attempted to gain telegraph at 4000 feet, 3960 feet, 3920 feet and 3880 feet. Chipco successfully gained telegraph and ripped casing at 3813 feet. Chipco tripped 4.5 inch diameter casing from wellbore and will trip 1.5 inch diameter back in to complete plugging operations tomorrow.

1056840149 2017-03-17 Plug / Plug Back No Violations On the day of my inspection I witnessed that Chipco had tripped 1.5 inch diameter tubing to a depth of 3793 feet. Hydrogen Sulfide (H2S) readings were at zero. Chipco cleared tubing and pumped a 60 sack 200 foot Class A Big Lime cement plug from 3793 feet to 3593 feet and displaced cement with 9 barrels of fresh water. Chipco tripped 1.5 inch diameter tubing up to 1469 feet. Chipco gained circulation and completed bottoms up of 22.3 barrels. Chipco pumped a 60 sack 200 foot Class A bottom of the surface cement plug from 1469 feet to 1269 feet and displaced cement with 3.2bbls of fresh water. Chipco tripped tubing to 200 feet, gained circulation and pumped an 85 sack 200 foot Class A cement plug from 200 foot to surface circulating cement to surface. Chipco tripped tubing out and topped off cement with cement staying at the surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.
