0188996581 2015-02-03 Production Wells No Violations On the day of my inspection following up on NOV-1123720868, I witnessed that the production equipment included a 210bbl tank which was shared with the 34059228860000 Bond 1, a horizontal separator, a 2 inch sales meter with a .75 inch orifice and a chart recorder. The well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing, a plunger lift system with valves open on the tubing and the flowline shut and disconnected. AEU is rigging up Nomac Drilling Rig #137 adjacent to well with plans to begin drilling in 30 days. I placed call to Enervest requesting an update and my concerns on well and Tim McConaha stated that he would see how soon Knox Energy could plug the well.
0212334407 2015-02-17 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had treated the well with 500 gallons of sulfur clear with minimal readings of 1ppm. Berry Well Service rigged up to pull the 1.5 inch tubing and found the tubing was parted at 1498 feet. Berry Well Service hauled the 1.5 inch tubing back to the yard and will be bringing back 2.875 inch tubing to begin to fish remainder of tubing from wellbore.

BWS will trip back in with fish and try to get over 1.5inch tubing again in the morning. Readings taken with ODNR Honeywell PhD6 meter revealed no H2S at the time of inspection.

Readings taken with ODNR Honeywell PhD6 meter revealed no H2S at the time of inspection.

Appalachian Well Service was on site and ran down inside 1.5 inch tubing and had trouble several spots before setting down at 3152 feet. Appalachian Well Service made multiple attempts to getOn the day of my inspection I witnessed Berry Well Service had tripped back in with a 3.625 inch fish with a 2.20 grapple to a depth of 1607 feet. Appalachian Well Service was on site and ran down inside 1.5 inch tubing and had trouble several spots before setting down at 3152 feet. Appalachian Well Service made multiple attempts to get their tool down the tubing past 3152 feet and then shot the tubing off 3130 feet. BWS tripped pipe out of the hole, retrieved fish and will trip 2.375 inch tubing back in preparation for cementing tomorrow.

Tim McConaha of Enervest requested to leave 8.625 inch wellhead in place for a few weeks incase cement would drop so that it could be topped off again before final restoration is completed. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.
2036122153 2015-03-11 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed Berry Well Service had tripped back in with 2.375 inch tubing to 3087 feet. Appalachian Well Service was on site and perforated 4.5 inch casing at with 10-1 11/16 inch shots from 3120 feet to 3110 feet. Petroset was on site and pumped a 400 foot 100 sack Class A cement with 2% Gel plug from 3091 feet to 2691 feet through 2.375 inch tubing. BWS tripped out the 2.375 inch tubing and pulled 4.5 inch casing which was parted at 1498 feet.
2039744385 2015-03-12 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had retrieved 1498 feet of 4.5 inch casing where it was parted and tripped back in with 2.375 inch tubing to 1496 feet. Petroset was on site and pumped a 300 foot 80 sack Class A cement bottom of the Surface casing plug from 1491 feet to 1191 feet through 2.375 inch tubing. BWS pulled 41 joint s of 2.375 inch tubing and Petroset pumped a 75 sack 200 foot Class A cement Surface plug from 200 feet to surface. BWS pulled remaining 7 joints of tubing and cement dropped and Petroset topped off with 35 sacks. Cement dropped again to 19 feet below surface and BWS was instructed to check level of plug tomorrow and let ODNR know level so I can be present for additional top off. NOV-1123720868 is resolved
