0386067020 2016-03-22 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a production tree with valves in the open position and a gauge present read 705psi on the casing. Production equipment included a sand trap, a line heater and a GP Unit with a 4 inch sales meter with a 2 inch orifice and chart recorder. Also present an oil stabilizer unit, a vapor recovery system, 15-400bbl tanks all within containment and a gas combustor all shared with the 2H, 4H, 6H, 10H and the 12H
0551047743 2017-02-13 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with a production tree with valves in the open position and a gauge present read 480 pounds per square inch on the casing with annular pressure monitoring in place. Production equipment included a sand trap, a line heater and a GP Unit with a 4 inch sales meter with a 2 inch orifice and chart recorder. Also present an oil stabilizer unit, a vapor recovery system, 15-400 barrel tanks all within containment and a gas combustor all shared with the 2H, 4H, 6H, 10H and the 12H.
0935119136 2018-07-24 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. A pressure gauge was present on all casing strings along with a pressure relief valve on the annulus. The production equipment consisted of six gas production units, one combustor, and fifteen 400-barrel tanks. A triethylene glycol dehydration unit with a contact tower and reboiler was located by the tank battery. The identification was located on the wellhead.
0995875714 2014-05-09 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed Universal Well Services break circulation with fresh water then displace six hundred and three sacks of Type 1 cement to a depth of 708 feet, I witnessed 22.0 sacks of cement returns to surface, site inspection revealed no issues.
1039148564 2015-02-25 Preliminary Restoration No Violations On the day of my inspection I witnessed that flow back has been completed and the well is online and equipped with a production tree with valves in the open position. Production equipment included a sand trap, a GP Unit, an oil stabilizer unit, a vapor recovery system and a total of 15-400bbl tanks all within containment. The site passed preliminary restoration according to ORC 1509.072A.
1095451829 2019-03-06 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of six gas production units, one triethylene glycol dehydration unit with a contact tower and reboiler, and fifteen 400-barrel tanks with vent lines plumbed to a compressor and one combustor. The identification was located on the wellhead.
1100629821 2016-04-28 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a production tree with valves in the open position and a gauge present read 660psi on the casing. Production equipment included a sand trap, a line heater and a GP Unit with a 4 inch sales meter with a 2 inch orifice and chart recorder. Also present an oil stabilizer unit, a vapor recovery system, 15-400bbl tanks all within containment and a gas combustor all shared with the 2H, 4H, 6H, 10H and the 12H.
1493345678 2016-06-20 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a production tree with valves in the open position and a gauge present read 640psi on the casing. I placed call to REX Energy EHS Supervisor Derrick Smith and I stated that valves were in place for annular monitoring but no gauges or relief valves. Mr. Smith stated that the work would be completed by the end of the week.
1602315251 2016-10-20 Production Wells No Violations On the day of my inspection I witnessed that annular monitoring is now in place. The well was equipped with a production tree with valves in the open position and a gauge present read 580 pounds per square inch on the casing.
1748892169 2014-06-19 Casing String Witnessed No Violations On the date of inspection I accompanied ODNR Inspector Mike Hartshorn to witness a surface casing string. When we arrived on site we were informed that the casing crew had damaged tools and would be unable to perform the cement string until later. Inspector Hartshorn and I left the site and would have to return when the casing was set correctly. Refer to Mike Hartshorns report for further details and witness.
