-362, I witnessed the well was shut-in as the order requires. Current chart indicates that well has not produced gas since Thursday July 30, 2015 at 7:15pm. I then placed call to REX Energy HSE Supervisor Derrick Smith and he stated that they shut-in the said well on Thursday and they will inform ODNR on how they will comply with order.

0366484293 2018-07-24 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. A pressure gauge was present on all casing strings along with a pressure relief valve on the annulus. The production equipment consisted of six gas production units, one combustor, and fifteen 400-barrel tanks. A triethylene glycol dehydration unit with a contact tower and reboiler was located by the tank battery. The identification was located on the wellhead.

The was equipped with a production tree with valves in the open position and gauges present read 675 pounds per square inch on the casing and 390psi on the tubing. Production equipment included a sand trap, a line heater and a GP Unit with a 4 inch sales meter with a 2 inch orifice and chart recorder. Also present an oil stabilizer unit, a vapor recovery system, 15-400 barrel tanks all within containment and a gas combustor all shared with the 2H, 4H, 8H, 10H and the 12H.

1000613751 2015-02-25 Preliminary Restoration No Violations On the day of my inspection I witnessed that flow back has been completed and the well is online and equipped with a production tree with valves in the open position. Production equipment included a sand trap, a GP Unit, an oil stabilizer unit, a vapor recovery system and a total of 15-400bbl tanks all within containment. The site passed preliminary restoration according to ORC 1509.072A.

1408305940 2014-08-17 Casing String Witnessed No Violations On the day of my inspection I witnessed that Sidewinder Drilling Rig #63 drilled a 17.5 inch diameter hole to the depth of 1320 feet. Kimzey Casing Services ran 1320 feet of 13.375 inch J-55 54.5#/ft. casing with the rig having to wash down the last joint of casing. Halliburton was on site and gained circulation with 150bbls of fresh water then pumped 50bbls of gel, 5bbls of fresh water and cemented casing with 1160 sacks of Class A cement with a 15.6ppg density and a yield of 1.21. Displaced cement with 196.5bbls of fresh water. Good returns of 75bbls of cement, plug was landed at 600psi and pressured up to 1200psi for 5 minutes and the floats held. Cement stayed to surface.
1607281429 2019-03-06 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of six gas production units, one triethylene glycol dehydration unit with a contact tower and reboiler, and fifteen 400-barrel tanks with vent lines plumbed to a compressor and one combustor. The identification was located on the wellhead.

-362, I witnessed that well was equipped with a production tree with valves in the open position and gauges present read 740psi on the casing and 390psi on the tubing. Production equipment included a sand trap, a line heater and a GP Unit with a 4 inch sales meter with a 2 inch orifice and chart recorder. Also present an oil stabilizer unit, a vapor recovery system, 15-400bbl tanks all within containment and a gas combustor all shared with the 2H, 4H, 8H, 10H and the 12H. I placed call to REX Energy EHS Supervisor Derrick Smith and I stated that valves were in place for annular monitoring but no gauges or relief valves. Mr. Smith stated that the work would be completed by the end of the week.

2067238188 2014-08-25 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Sidewinder Drilling Rig #63 had ran and set 9.625 inch intermediate casing and was picking up tools to drill out DV tool and float equipment. Site inspection revealed minor tears in containment with crew on site making repairs.
